Description
Overview of the medicine
Bromhexine is a mucolytic agent used to thin and loosen mucus in the airways, making it easier to cough up. It is primarily used for the treatment of respiratory conditions associated with excessive or thick mucus secretion.
Uses & Indications
Dosage
Adults
8 mg (10 ml) three times daily. May be increased to 16 mg (20 ml) three times daily in severe cases.
Elderly
No specific dosage adjustment usually required, use adult dose.
Renal_impairment
Caution, dose reduction may be needed in severe renal impairment. Consult a physician.
How to Take
Oral administration. Take with or without food. Use the provided measuring cup for accurate dosing.
Mechanism of Action
Bromhexine depolymerizes mucopolysaccharide fibers directly, reducing mucus viscosity and stimulating serous gland activity, leading to increased and easier expectoration. It also has secretomotor effects, enhancing mucociliary clearance.
Pharmacokinetics
Onset
Effects typically begin within 30 minutes to 1 hour.
Excretion
Primarily excreted via urine, mainly as metabolites.
Half life
Terminal half-life of the active metabolite is approximately 6.5 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver to various hydroxylated metabolites, including ambroxol, which is also pharmacologically active.
Side Effects
Contraindications
- Hypersensitivity to bromhexine or any component of the formulation.
- Patients with active peptic ulcer disease (relative contraindication due to potential for gastric irritation).
Drug Interactions
Cough suppressants
Concurrent use should be avoided as it may lead to accumulation of mucus due to impaired cough reflex, increasing the risk of respiratory obstruction.
Antibiotics (e.g., amoxicillin, erythromycin, oxytetracycline)
Bromhexine may increase the penetration of antibiotics into bronchial secretions, potentially enhancing their therapeutic effect.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, epigastric pain, and other gastrointestinal disturbances. Management is symptomatic and supportive. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Use with caution during pregnancy, especially during the first trimester. Consult a physician. Bromhexine passes into breast milk, so caution is advised during lactation. The benefits should outweigh the potential risks.

Lab Monitoring
- No routine lab monitoring is generally required for bromhexine.
- Monitor for liver function tests in patients with pre-existing hepatic impairment if long-term use is anticipated.
Doctor Notes
- Ensure patients understand the difference between productive and non-productive coughs for appropriate use.
- Advise patients about maintaining adequate hydration for optimal mucolytic effect.
- Counsel on the importance of adhering to prescribed dosage and not exceeding it.
- Be vigilant for rare but serious skin reactions (SCARs).
Patient Guidelines
- Drink plenty of fluids (water, juice) to help thin mucus and improve expectoration.
- Do not use for persistent or chronic cough without consulting a doctor.
- Report any severe skin reactions (e.g., rash with fever, blistering) immediately to your doctor.
- Avoid concurrent use with cough suppressants.
- Complete the full course as prescribed by your doctor, even if symptoms improve.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Brolyt is generally not expected to affect the ability to drive or operate machinery. However, if side effects like dizziness or headache occur, patients should avoid driving or operating heavy machinery.
Lifestyle Advice
- Maintain good hydration throughout the day.
- Avoid smoking and exposure to secondhand smoke, as it can worsen respiratory conditions.
- Ensure adequate rest to aid recovery.
- Humidifiers can help moisten air and ease breathing.
