Buticef 400 mg Capsule contains Ceftibuten, a third-generation cephalosporin antibiotic, effective against a wide range of gram-positive and gram-negative bacteria. It is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily for 5-10 days, depending on the type and severity of infection.
Elderly
No dose adjustment generally required, but close monitoring of renal function is advised, especially in severe renal impairment.
Renal_impairment
For creatinine clearance < 50 mL/min, dosage adjustment is necessary. For example, for CrCl 30-49 mL/min, 200 mg once daily. For CrCl < 29 mL/min, 100 mg once daily or 400 mg every 48 hours.
How to Take
Buticef 400 mg Capsule should be taken orally. It can be administered with or without food. It is recommended to take it at approximately the same time each day to maintain consistent drug levels.
Mechanism of Action
Ceftibuten, like other cephalosporins, exerts its bactericidal action by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), which are enzymes involved in the final steps of peptidoglycan synthesis, a crucial component of the bacterial cell wall. This binding prevents cross-linking of peptidoglycan, leading to a defective cell wall and ultimately bacterial cell lysis and death.

Primarily excreted unchanged in urine via glomerular filtration.

Approximately 2-3 hours.
Absorption
Well absorbed orally. Peak plasma levels usually reached within 2-3 hours. Food does not significantly affect absorption.

Minimally metabolized; primarily converted to an active trans-isomer metabolite.
Side Effects
Contraindications
- Known hypersensitivity to Ceftibuten or to any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics due to potential cross-reactivity.
Drug Interactions
Antacids
May decrease the absorption of Ceftibuten if taken concurrently. Administer Ceftibuten at least 2 hours before or 4 hours after antacids containing aluminum or magnesium.
Probenecid
May increase Ceftibuten plasma levels and prolong its half-life by decreasing renal tubular secretion.
Oral Anticoagulants (e.g., Warfarin)
Although not consistently demonstrated, some cephalosporins may alter vitamin K metabolism and potentiate the effect of anticoagulants; monitor INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, abdominal discomfort, and potentially neurological effects like convulsions in very high doses, especially with renal impairment. Treatment should be symptomatic and supportive. Gastric lavage may be considered. Hemodialysis may be useful in reducing serum levels, particularly in patients with severely impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Animal studies have not shown evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Buticef should be used during pregnancy only if clearly needed. Ceftibuten is excreted in low concentrations in human breast milk; therefore, caution should be exercised when Buticef is administered to a nursing woman.
