Caboxen 20 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like heartburn, acid reflux, stomach ulcers, and other acidity-related issues.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For Duodenal Ulcer: 20 mg once daily for 2-4 weeks. For Gastric Ulcer: 20 mg once daily for 4-8 weeks. Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted as needed.
Elderly
No dosage adjustment is generally needed for the elderly with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is generally needed for patients with renal impairment.
How to Take
Take Caboxen 20 mg Capsule orally, preferably in the morning before a meal. Swallow the capsule whole with water; do not chew or crush it. If swallowing is difficult, the capsule may be opened and the pellets mixed with a small amount of fruit juice or applesauce.
Mechanism of Action
Omeprazole inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step of acid production. This leads to a dose-dependent inhibition of both basal and stimulated acid secretion.

Onset
Within 1 hour
Excretion
Mainly excreted in urine as metabolites (approx. 77%) and in feces (approx. 18%).
Half life
0.5 to 1 hour (plasma), but effects on acid suppression are much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract, primarily in the small intestine. Bioavailability is about 30-40% after a single dose, increasing to about 60% with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation.
- Concomitant use with Nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time. Monitor frequently.
Tacrolimus
Increased tacrolimus levels. Monitor drug levels.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of Clopidogrel. Avoid concomitant use.
Methotrexate
Increased methotrexate levels. Consider temporary withdrawal of omeprazole.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, blurred vision, tachycardia, nausea, sweating, headache, dry mouth. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Excreted in breast milk, caution is advised in nursing mothers.
