Description
Overview of the medicine
Cabozantinib is an anti-cancer medication that works by inhibiting multiple tyrosine kinases involved in tumor growth, angiogenesis, and metastasis. It is used to treat various types of cancer, including medullary thyroid cancer, renal cell carcinoma, and hepatocellular carcinoma.
Uses & Indications
Dosage
Adults
For MTC and RCC: 140 mg (one 80 mg and three 20 mg capsules) orally once daily. For HCC and DTC: 60 mg (one 60 mg capsule) orally once daily. Doses should be adjusted based on tolerability.
Elderly
No specific dose adjustment is required based on age alone, but monitor for adverse reactions due to potential decreased organ function.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. No data for severe renal impairment.
How to Take
Take orally with or without food. Do not eat for at least 2 hours before and for at least 1 hour after taking Cabozantinib. Swallow capsules whole; do not open, crush, or chew. Do not take two doses in the same day to make up for a missed dose.
Mechanism of Action
Cabozantinib is a tyrosine kinase inhibitor (TKI) that targets multiple receptor tyrosine kinases (RTKs) including MET, VEGFR1, VEGFR2, VEGFR3, AXL, RET, ROS1, TYRO3, MER, and KIT. By inhibiting these kinases, it disrupts tumor angiogenesis, proliferation, and survival.
Pharmacokinetics
Onset
Not explicitly defined for clinical effect, but pharmacodynamic effects on target inhibition occur within hours.
Excretion
Mainly excreted in feces (approximately 81%) with a minor portion in urine (approximately 8%).
Half life
Approximately 99 hours (range 86-111 hours), indicating once-daily dosing.
Absorption
Oral bioavailability is moderate to high. Peak plasma concentrations are reached 2-4 hours after administration.
Metabolism
Primarily metabolized by CYP3A4. Other minor pathways involve UGT-mediated glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to cabozantinib or any excipients.
- Patients with uncontrolled hypertension, recent bleeding events, or gastrointestinal perforation.
Drug Interactions
CYP3A4 inducers (e.g., rifampin, phenytoin)
Decrease cabozantinib exposure; increase cabozantinib dose.
CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
Increase cabozantinib exposure; reduce cabozantinib dose.
Proton pump inhibitors (PPIs), H2-receptor antagonists
May decrease absorption of cabozantinib (avoid co-administration or monitor).
Storage
Store at room temperature (20°C to 25°C), excursions permitted to 15°C to 30°C. Keep bottle tightly closed. Protect from moisture and light. Keep out of reach of children.
Overdose
No specific antidote for cabozantinib overdose. Manage overdose with symptomatic and supportive care. Closely monitor patients for adverse events and initiate appropriate treatment.
Pregnancy & Lactation
Pregnancy Category D. Can cause fetal harm. Advise females of reproductive potential to use effective contraception during treatment and for at least 4 months after the last dose. It is unknown if cabozantinib is excreted in human milk; due to potential for serious adverse reactions in breastfed infants, advise lactating women not to breastfeed during treatment and for 4 months after the last dose.

Doctor Notes
- Careful monitoring for hypertension, GI toxicities (diarrhea, perforation), and hemorrhage is crucial.
- Dose modifications based on adverse event severity are frequently required.
- Consider patient's overall health, comorbidities, and concomitant medications before initiating therapy.
Patient Guidelines
- Report any signs of bleeding (e.g., black tarry stools, severe headaches) immediately.
- Monitor blood pressure regularly and report significant elevations.
- Avoid prolonged sun exposure and use sunscreen.
