Description
Overview of the medicine
Canalia 30 mg Capsule is an oral antidiabetic medication used to improve glycemic control in adults with type 2 diabetes mellitus. It belongs to a class of drugs called SGLT2 inhibitors, which help the kidneys remove glucose from the body through urine, thereby lowering blood sugar levels. It may also offer cardiovascular and renal benefits.
Uses & Indications
Dosage
Adults
The recommended starting dose is 30 mg orally once daily, taken before the first meal of the day. The dose may be increased to 60 mg once daily based on glycemic response and tolerability. Do not exceed 60 mg daily.
Elderly
No dose adjustment is recommended based on age alone. However, renal function should be assessed before initiating Canalia and periodically thereafter, as elderly patients are more likely to have impaired renal function.
Renal_impairment
For patients with eGFR 45 to <60 mL/min/1.73 m², the starting dose is 30 mg once daily. For eGFR <45 mL/min/1.73 m², initiation is not recommended. If eGFR falls below 45 mL/min/1.73 m² during treatment, Canalia should be discontinued.
How to Take
Take Canalia capsule orally once daily in the morning, before the first meal. Swallow the capsule whole with water. Do not crush, chew, or break the capsule.
Mechanism of Action
Canaglifil selectively inhibits sodium-glucose co-transporter 2 (SGLT2) in the renal proximal tubules, reducing glucose reabsorption and increasing urinary glucose excretion (glucosuria). This leads to a reduction in plasma glucose concentrations.
Pharmacokinetics
Onset
Glycemic effect usually begins within a few hours of the first dose.
Excretion
Approximately 33% excreted in urine (primarily as metabolites) and 60% in feces (mostly as unchanged drug).
Half life
Approximately 10-13 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily metabolized by O-glucuronidation via UGT1A9 and UGT2B4 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Canaglifil or any excipients in the capsule.
- Severe renal impairment, end-stage renal disease (ESRD), or patients on dialysis.
Drug Interactions
Digoxin
Canalia may increase the exposure to digoxin. Digoxin levels should be monitored.
Rifampin
May decrease the efficacy of Canalia.
Diuretics
May increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues
May increase the risk of hypoglycemia. A lower dose of insulin or secretagogue may be required.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of sight and reach of children.
Overdose
In case of an overdose, contact a poison control center or emergency room immediately. Symptomatic and supportive treatment should be initiated. Canaglifil is minimally dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Canalia is not recommended during pregnancy, especially during the second and third trimesters. It is unknown whether Canaglifil is excreted in human milk; therefore, it is not recommended during breastfeeding.

Clinical Trials
Canaglifil underwent extensive Phase III clinical trials demonstrating its efficacy and safety in improving glycemic control and reducing cardiovascular and renal risks in patients with type 2 diabetes mellitus.
Lab Monitoring
- Renal function (eGFR) should be assessed before initiation and periodically thereafter.
- Glycemic control (HbA1c, fasting plasma glucose) should be monitored regularly.
- Lipid profile (though less direct effect).
Doctor Notes
- Emphasize patient education on hydration and hygiene to mitigate infection risks.
- Regularly monitor renal function and adjust dosage as per eGFR guidelines.
- Advise patients to seek immediate medical attention for symptoms suggestive of DKA or Fournier's gangrene.
Patient Guidelines
- Take Canalia exactly as prescribed by your doctor.
- Stay well-hydrated, especially in hot weather or during illness, to prevent dehydration.
- Monitor your blood glucose levels regularly and report any unusual symptoms to your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Canalia alone is not expected to cause hypoglycemia, so it is unlikely to affect your ability to drive or operate machinery. However, if taken with insulin or sulfonylureas, the risk of hypoglycemia may increase, which could impair driving ability.
Lifestyle Advice
- Maintain a balanced diet and engage in regular physical activity as part of your diabetes management plan.
- Avoid excessive alcohol consumption, as it can affect blood sugar levels.
- Practice good hygiene to reduce the risk of genital and urinary tract infections.
