Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours. Duration typically 7-14 days depending on the infection. For uncomplicated gonorrhea, a single 400 mg dose.
Elderly
No specific dose adjustment is generally required unless there is severe renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 200 mg daily. For creatinine clearance < 20 mL/min, the dose should be reduced to 100 mg daily.
How to Take
Cefim-3-DS 400 mg capsule can be taken orally with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.

Excretion
Primarily renal excretion (approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours). Some biliary excretion also occurs.

Absorption
Approximately 40-50% orally absorbed. Peak plasma concentrations are reached 2-6 hours after administration. Food may delay absorption but does not significantly affect the total amount absorbed.
Metabolism
Minimal hepatic metabolism. No active metabolites identified.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
Increases cefixime plasma concentrations and extends its half-life by inhibiting renal excretion.
Carbamazepine
Cefixime may increase carbamazepine levels when co-administered. Monitor carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time (INR) and the risk of bleeding. Close monitoring of coagulation parameters is recommended.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Small amounts of cefixime are excreted in breast milk; use with caution during lactation.
