Overview of the medicine
Ciprofloxacin is a broad-spectrum antibiotic from the fluoroquinolone class. It is used to treat a wide range of bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, bone and joint infections, and certain gastrointestinal infections.
Uses & Indications
Dosage
Adults
The usual adult oral dose is 250 mg to 750 mg twice daily, depending on the severity and type of infection. For most moderate infections, 500 mg twice daily is common. Duration varies from 7 to 14 days.
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be assessed, and dosage adjusted accordingly if renal impairment is present.
Renal_impairment
Dosage adjustment is necessary for patients with impaired renal function (creatinine clearance < 30 mL/min). A common recommendation is half the usual dose or extending the dosing interval.
How to Take
Take the tablet orally with a glass of water, with or without food. Do not take with dairy products (milk, yogurt) or calcium-fortified juices alone. Avoid taking antacids, sucralfate, or iron/zinc supplements within 2 hours of taking Ciprofloxacin.
Mechanism of Action
Ciprofloxacin works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.

Therapeutic effects typically begin within 1-2 hours after administration.
Excretion
Primarily excreted via the kidneys (approximately 50-70% as unchanged drug) and to a lesser extent via feces (approximately 15% as unchanged drug).
Half life
Plasma elimination half-life is typically 3-5 hours.
Absorption
Rapidly and well-absorbed from the gastrointestinal tract after oral administration. Bioavailability is approximately 70-80%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Approximately 15-30% of Ciprofloxacin is metabolized in the liver to several less active metabolites, primarily via CYP1A2.
Side Effects
Contraindications
- Hypersensitivity to Ciprofloxacin or other fluoroquinolones
- Concomitant use with Tizanidine (due to risk of severe hypotension and somnolence)
- History of tendon disorders related to quinolone use
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk. Monitor INR/PT.
Tizanidine
Contraindicated due to significant increase in Tizanidine exposure, leading to hypotension and somnolence.
Cyclosporine
May increase serum creatinine levels. Monitor renal function.
Theophylline
Ciprofloxacin can increase theophylline plasma levels, leading to increased risk of toxicity. Monitor theophylline levels.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
Concurrent use may increase the risk of CNS stimulation and convulsive seizures.
Antacids (containing aluminum/magnesium), Sucralfate, Didanosine, Iron/Zinc salts
Can significantly reduce Ciprofloxacin absorption. Administer Ciprofloxacin at least 2 hours before or 6 hours after these agents.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In the event of an acute overdose, gastric emptying (e.g., gastric lavage, induced emesis) may be considered. Maintain adequate hydration. Symptomatic and supportive care should be instituted. Ciprofloxacin is not effectively removed by hemodialysis or peritoneal dialysis.
Pregnancy & Lactation
Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk and may cause adverse effects in a nursing infant. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
