Description
Overview of the medicine
Clinacyn 150 mg Capsule contains Clindamycin, a lincosamide antibiotic, used to treat a wide range of bacterial infections including those of the skin, bone, joints, lungs, abdomen, and female reproductive organs.
Uses & Indications
Dosage
Adults
150-300 mg every 6-8 hours, depending on the severity of infection. Maximum 450 mg every 6-8 hours for severe infections.
Elderly
No specific dose adjustment is usually required based on age alone; monitor renal/hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe impairment, consider reducing dose or extending dosing interval.
How to Take
Take orally with a full glass of water, with or without food, to prevent esophageal irritation. Do not lie down for at least 30 minutes after taking.
Mechanism of Action
Clindamycin binds to the 50S ribosomal subunit of susceptible bacteria, inhibiting bacterial protein synthesis. This action makes it primarily bacteriostatic, although it can be bactericidal at higher concentrations or against highly susceptible organisms.
Pharmacokinetics
Onset
Effect typically seen within a few hours of administration.
Excretion
Approximately 10-20% excreted in urine and 4% in faeces as active drug, with the remainder excreted as metabolites.
Half life
Approximately 2-3 hours in adults with normal renal and hepatic function. Can be prolonged in patients with severe renal or hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (about 90%) after oral administration. Peak plasma concentrations are reached within 45-60 minutes.
Metabolism
Primarily metabolized in the liver to active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to clindamycin, lincomycin, or any component of the formulation
- History of antibiotic-associated colitis
Drug Interactions
Erythromycin
Concurrent use may result in antagonism as both drugs bind to the 50S ribosomal subunit.
Kaolin-pectin antidiarrheals
May reduce the absorption of oral clindamycin. Administer clindamycin at least 2 hours before these agents.
Neuromuscular blocking agents
Clindamycin may potentiate the action of these agents, leading to prolonged neuromuscular blockade.
Storage
Store below 30°C in a cool, dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal distress. Management is supportive and symptomatic. Hemodialysis and peritoneal dialysis are not effective in removing clindamycin from the blood.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Clindamycin is excreted in breast milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Periodic liver and kidney function tests for prolonged therapy
- Complete blood count (CBC) for prolonged therapy
- Monitor for signs and symptoms of C. difficile-associated diarrhea
Doctor Notes
- Counsel patients about the risk of C. difficile-associated diarrhea and to report persistent diarrhea immediately.
- Advise patients to take with plenty of water to minimize esophageal irritation.
- Consider alternative antibiotics if patient has a history of severe gastrointestinal issues with prior antibiotic use.
Patient Guidelines
- Complete the full course of antibiotics, even if symptoms improve.
- Take with a full glass of water to prevent throat irritation.
- Report any severe diarrhea, abdominal pain, or bloody stools immediately.
- Do not use for viral infections.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Generally, clindamycin does not impair the ability to drive or operate machinery. However, if side effects like dizziness occur, avoid such activities.
Lifestyle Advice
- Maintain good hygiene to prevent infections.
- Consume probiotics (e.g., yogurt) to help maintain gut flora balance during antibiotic use, but not at the same time as the antibiotic.
