Description
Overview of the medicine
Cloxacillin is a penicillin antibiotic used to treat bacterial infections, particularly those caused by penicillinase-producing staphylococci. It is effective against various skin, soft tissue, respiratory, and bone infections.
Uses & Indications
Dosage
Adults
Not applicable for pediatric drops. Adults typically use capsules or tablets.
Elderly
Not applicable for pediatric drops. Elderly patients using other forms may require renal dosage adjustment.
Renal_impairment
In severe renal impairment (creatinine clearance < 10 mL/min), dosage reduction or extended dosing interval may be necessary. Consult a physician.
Pediatric_general
For children under 2 years: 62.5 mg (2.5 mL) every 6 hours. For children 2-10 years: 125 mg (5 mL) every 6 hours. Doses may be increased in severe infections, as directed by a physician.
How to Take
Administer orally. Shake the bottle well before each use. Measure the dose accurately using the provided dropper or measuring spoon. It is best taken on an empty stomach (1 hour before or 2 hours after meals) for optimal absorption, but can be taken with food to minimize gastrointestinal upset.
Mechanism of Action
Cloxacillin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Rapid
Excretion
Primarily via renal excretion (urine) and bile.
Half life
0.5-1.5 hours in adults, may be prolonged in neonates.
Absorption
Well absorbed orally, but food reduces absorption. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to penicillin antibiotics or any component of the formulation.
- History of severe allergic reaction (e.g., anaphylaxis, Stevens-Johnson syndrome) to any beta-lactam antibiotic.
Drug Interactions
Warfarin
May increase the anticoagulant effect of warfarin, requiring monitoring of INR.
Probenecid
Increases and prolongs plasma concentrations of cloxacillin by inhibiting renal tubular secretion.
Methotrexate
Cloxacillin may decrease the renal excretion of methotrexate, leading to increased methotrexate toxicity.
Tetracyclines/Chloramphenicol
These bacteriostatic antibiotics may antagonize the bactericidal effect of penicillin.
Storage
Store the dry powder below 30°C, protected from light and moisture. Once reconstituted, store the suspension in a refrigerator (2-8°C) and discard after the recommended period (usually 7-14 days). Do not freeze.
Overdose
Overdose is generally associated with gastrointestinal symptoms (nausea, vomiting, diarrhea) and electrolyte imbalance. In very high doses, CNS excitation and convulsions may occur. Treatment is symptomatic and supportive; no specific antidote exists. Hemodialysis may be beneficial in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have shown no risk, but human data are limited. Use only if clearly needed. Cloxacillin is excreted in small amounts in breast milk; use with caution during lactation, monitoring the infant for adverse effects like diarrhea or candidiasis.

YesClinical Trials
Cloxacillin has been extensively studied since its introduction in 1965, with numerous clinical trials establishing its efficacy and safety profile in treating staphylococcal infections.
Lab Monitoring
- Liver function tests (LFTs) for prolonged therapy or in patients with hepatic impairment.
- Renal function tests (e.g., creatinine, BUN) in patients with pre-existing renal impairment or on high doses.
- Complete blood count (CBC) during prolonged therapy.
Doctor Notes
- Emphasize the importance of completing the full prescribed course to prevent antibiotic resistance and ensure complete eradication of the infection.
- Educate parents on proper reconstitution, accurate measurement, and storage of the pediatric drop.
- Monitor for signs of allergic reactions, especially in patients with a history of penicillin allergy.
Patient Guidelines
- Complete the entire course of medication as prescribed by the doctor, even if symptoms improve, to prevent recurrence and antibiotic resistance.
- Shake the bottle well before administering each dose.
- Use the provided measuring device to ensure accurate dosing for your child.
- Store the reconstituted drop in the refrigerator and discard any unused portion after the recommended period (usually 7-14 days).
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your child's next dose. In that case, skip the missed dose and continue with the regular dosing schedule. Do not double the dose.
Driving Precautions
Cloxacillin is generally not known to impair the ability to drive or operate machinery. However, if side effects like dizziness occur, caution should be advised.
Lifestyle Advice
- Encourage your child to stay well-hydrated during treatment.
- Maintain good hygiene practices to prevent the spread of infection.
