Description
Overview of the medicine
Cloram 1% Eye Ointment contains Chloramphenicol, an antibiotic used to treat bacterial infections of the eye including bacterial conjunctivitis, keratitis, and blepharitis.
Uses & Indications
Dosage
Adults
Apply a small amount (approximately 1 cm ribbon) into the conjunctival sac 3-4 times daily, or more frequently if required, for 5-7 days or as directed by a physician.
Elderly
No special dosage adjustment is generally required.
Renal_impairment
No special dosage adjustment is generally required for topical ophthalmic use.
How to Take
Wash hands thoroughly before and after application. Gently pull down the lower eyelid to form a pocket. Apply a thin strip of ointment into the pocket. Close the eye gently for a few seconds. Do not touch the tip of the tube to the eye or any other surface.
Mechanism of Action
Chloramphenicol is a broad-spectrum antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing peptide bond formation and bacterial growth.
Pharmacokinetics
Onset
Within hours (local effect)
Excretion
Excreted mainly in the urine as inactive metabolites if systemically absorbed.
Half life
Not well-defined for topical ophthalmic use; systemic half-life is 1.6-3.3 hours.
Absorption
Following topical ophthalmic administration, chloramphenicol penetrates the cornea and aqueous humor. Systemic absorption is minimal but can occur.
Metabolism
Primarily metabolized in the liver (glucuronidation) if systemically absorbed.
Side Effects
Contraindications
- Hypersensitivity to chloramphenicol or any component of the formulation.
- History of bone marrow depression or blood dyscrasias associated with chloramphenicol use.
Drug Interactions
No significant drug interactions have been reported for topical ophthalmic chloramphenicol.
However, systemic absorption is possible, albeit minimal, so caution should be exercised with drugs that may affect bone marrow or are metabolized by hepatic enzymes.
Storage
Store below 25°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Accidental ingestion of the eye ointment is unlikely to cause systemic toxicity due to the small amount of drug. Topical overdose may lead to increased local side effects such as stinging or irritation. Management should be supportive and symptomatic.
Pregnancy & Lactation
Chloramphenicol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother. Consult a physician.
