Description
Overview of the medicine
Bromocriptine is a dopamine agonist used in the treatment of various conditions including Parkinson's disease, hyperprolactinemia (e.g., amenorrhea, galactorrhea, infertility), acromegaly, and type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Dosage varies widely by indication. For hyperprolactinemia, initial dose typically 1.25 mg at bedtime, increasing gradually to 2.5-15 mg/day. For Parkinson's, initial dose 1.25 mg once or twice daily, titrated up to 20-100 mg/day depending on response. Always start low and titrate slowly.
Elderly
Start with lower doses and titrate gradually due to increased sensitivity to side effects.
Renal_impairment
No specific dosage adjustment for mild to moderate renal impairment, but caution is advised. Not studied in severe renal impairment.
How to Take
Take orally, usually with food to minimize gastrointestinal upset. Swallow tablets whole with water.
Mechanism of Action
Stimulates postsynaptic dopamine D2 receptors, inhibits prolactin secretion from the pituitary gland, and reduces growth hormone secretion in acromegaly.
Pharmacokinetics
Onset
Prolactin-lowering effects within 1-2 hours; peak plasma concentration in 1-3 hours.
Excretion
Mainly excreted via bile and feces (85%), with a small amount (5%) excreted renally.
Half life
Biphasic elimination with a terminal half-life of approximately 4.5-5 hours.
Absorption
Well absorbed orally, but undergoes extensive first-pass metabolism; bioavailability is about 28%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4).
Side Effects
Contraindications
- Uncontrolled hypertension
- Pre-eclampsia, eclampsia, or postpartum hypertension
- Hypersensitivity to bromocriptine or other ergot alkaloids
- Ischemic heart disease or severe peripheral vascular disease (for some indications)
Drug Interactions
Ergot alkaloids
Increased risk of adverse cardiovascular events.
Sympathomimetics
May increase the risk of hypertension.
Dopamine antagonists (e.g., antipsychotics)
May reduce the effectiveness of bromocriptine.
CYP3A4 inhibitors (e.g., erythromycin, ketoconazole)
Can increase bromocriptine plasma levels and adverse effects.
Storage
Store below 30°C (86°F), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, orthostatic hypotension, hallucinations, and confusion. Management is supportive, focusing on symptomatic treatment and maintaining cardiovascular stability.
Pregnancy & Lactation
Contraindicated during pregnancy for the inhibition of physiological lactation. For other indications, use only if clearly needed and benefits outweigh risks. Bromocriptine inhibits lactation and should be avoided in nursing mothers.

Lab Monitoring
- Prolactin levels (for hyperprolactinemia)
- Growth hormone/IGF-1 levels (for acromegaly)
- Blood pressure (especially postpartum and with dose titration)
- Renal and hepatic function tests (periodically)
Doctor Notes
- Emphasize slow dose titration to minimize dopaminergic side effects.
- Monitor blood pressure regularly, particularly in postpartum patients.
- Counsel patients on orthostatic hypotension and potential for somnolence.
- Consider drug interactions, especially with CYP3A4 inhibitors and dopamine antagonists.
Patient Guidelines
- Take with food to reduce nausea and stomach upset.
- Stand up slowly to avoid dizziness from orthostatic hypotension.
- Report any unusual side effects, especially hallucinations, confusion, or severe headache.
- Avoid alcohol while taking this medication.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next scheduled dose. Do not double doses.
Driving Precautions
Bromocriptine may cause dizziness, somnolence, or sudden onset of sleep. Patients should be cautioned about operating hazardous machinery, including automobiles, until they are reasonably certain that bromocriptine therapy does not affect them adversely.
Lifestyle Advice
- Maintain adequate hydration.
- For Parkinson's patients, adhere to a regular medication schedule for optimal symptom control.
