D-Fix 200 mg capsule contains Cefixime, a third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg daily, given as a single dose or in two divided doses (200 mg every 12 hours).
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function. Dose adjustment is necessary for elderly patients with renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, the dose should be reduced to 300 mg daily. For creatinine clearance <20 mL/min, the dose should be reduced to 200 mg daily.
How to Take
D-Fix capsules should be taken orally, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for bacterial cell wall integrity, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours. The remainder is excreted by non-renal mechanisms, likely biliary excretion.
Half life
Average plasma half-life is 3 to 4 hours, which may be prolonged in renal impairment.
Absorption
Approximately 40-50% orally absorbed. Food slows absorption but does not significantly affect the extent of absorption.
Metabolism
Not extensively metabolized in the body. No active metabolites identified.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase Cefixime bioavailability.
Carbamazepine
Elevated carbamazepine levels have been reported when administered concomitantly with Cefixime. Therapeutic drug monitoring may be useful.
Oral contraceptives
Some cephalosporins may reduce the efficacy of oral contraceptives, although data for Cefixime is limited.
Warfarin and other anticoagulants
May increase prothrombin time (PT) with or without bleeding, so careful monitoring of coagulation parameters is advised.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human breast milk. Caution should be exercised when Cefixime is administered to a nursing mother.
