Description
Overview of the medicine
D-gest 10 mg Tablet contains Dydrogesterone, a synthetic progestogen that is chemically similar to natural progesterone. It is used in conditions associated with progesterone deficiency.
Uses & Indications
Dosage
Adults
For dysfunctional uterine bleeding: 10 mg twice daily for 5-7 days. For threatened abortion: 40 mg initially, then 10 mg every 8 hours until symptoms subside. For habitual abortion: 10 mg twice daily until the 20th week of pregnancy.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with hepatic or renal impairment.
Renal_impairment
Use with caution; no specific guidelines for dose adjustment available. Monitor patient closely.
How to Take
Oral administration. Tablets should be swallowed whole with water, with or without food.
Mechanism of Action
Dydrogesterone acts directly on the uterus, mimicking the effects of natural progesterone. It helps to convert the proliferative endometrium into a secretory endometrium, preventing endometrial hyperplasia. It does not possess estrogenic, androgenic, anabolic, or corticosteroid activity.
Pharmacokinetics
Onset
Within hours for hormonal effects.
Excretion
Primarily excreted in the urine (about 60-70%) as metabolites within 72 hours, with some fecal excretion.
Half life
The half-life of dydrogesterone is approximately 5 to 7 hours, and for its main metabolite, 20-α-dihydrodydrogesterone, it is 14 to 17 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 0.5 to 2.5 hours.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation, to its main active metabolite, 20-α-dihydrodydrogesterone.
Side Effects
Contraindications
- Known hypersensitivity to dydrogesterone or any component of the tablet
- Undiagnosed vaginal bleeding
- Known or suspected progestogen-dependent tumour
- Severe hepatic impairment
Drug Interactions
Mifepristone
Concomitant use is not recommended as mifepristone is an anti-progestogen.
Enzyme inducers (e.g., phenobarbital, rifampicin, phenytoin)
May increase the metabolism of dydrogesterone, potentially reducing its effectiveness.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited data on overdose. Symptoms are generally mild and may include nausea, vomiting, dizziness. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Dydrogesterone is used to support pregnancy in cases of progesterone deficiency. Consult a doctor before use during lactation, as dydrogesterone and its metabolites are excreted in breast milk.
