Uses & Indications
Dosage
Adults
10 mg three to four times daily, before meals and at bedtime. Maximum daily dose is 30 mg.
Elderly
No specific dose adjustment for elderly patients, but caution is advised due to potential for cardiac effects.
Renal_impairment
In severe renal impairment (creatinine clearance < 30 mL/min), frequency should be reduced to once or twice daily depending on severity. Cardiac risks should be monitored.
How to Take
Take orally, preferably 15-30 minutes before meals. Do not crush or chew the tablet; swallow whole with water.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2 receptor antagonist, primarily blocking dopamine receptors in the chemoreceptor trigger zone (CTZ) located outside the blood-brain barrier. This action reduces nausea and vomiting. It also enhances gastrointestinal peristalsis by blocking dopamine receptors in the gut, promoting gastric emptying and intestinal transit.

Contraindications

- Coadministration with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
- Prolactinoma
- Patients with gastrointestinal hemorrhage, mechanical obstruction, or perforation
Drug Interactions
Antacids and H2-receptor blockers
May decrease the oral bioavailability of domperidone. Take domperidone before meals and antacids after meals.
QT-prolonging drugs (e.g., Amiodarone, Cisapride, Fluoxetine)
Increased risk of QT prolongation and cardiac arrhythmias. Avoid concomitant use.
Potent CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin, Ritonavir)
Increased plasma levels of domperidone, increased risk of QT prolongation. Concomitant use is contraindicated.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, disorientation, extrapyramidal reactions, and QT prolongation. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. ECG monitoring is recommended.
