Overview of the medicine
Deprolin 20 mg syrup contains Fluoxetine, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, and bulimia nervosa. It helps restore the balance of serotonin in the brain.
Uses & Indications
Dosage
Adults
Major Depressive Disorder: Initially 20 mg once daily, may be increased after several weeks to a maximum of 80 mg/day. OCD: Initially 20 mg once daily, typically 20-60 mg/day.
Elderly
Lower initial doses (e.g., 10 mg/day) may be considered, with gradual titration. Maximum recommended dose generally lower.
Renal_impairment
No dosage adjustment necessary for mild to moderate impairment. Use with caution in severe impairment; consider lower frequency/dose.
How to Take
Take orally, with or without food, usually once daily in the morning. For children, the syrup can be mixed with a small amount of water or juice if needed.
Mechanism of Action
Fluoxetine selectively inhibits the reuptake of serotonin by neurons in the central nervous system, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission.

Onset
Initial antidepressant effects may be seen within 1-2 weeks, but full therapeutic effect usually takes 4-5 weeks.
Excretion
Primarily excreted renally (about 60%) as metabolites, with a small amount excreted in feces.

Metabolism
Extensively metabolized in the liver by CYP2D6 to its active metabolite, norfluoxetine.
Side Effects
Contraindications
- Hypersensitivity to Fluoxetine or any component of the syrup
- Concurrent use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI
- Concurrent use with Thioridazine or Pimozide
Drug Interactions
MAOIs
Risk of serotonin syndrome, may be fatal. Avoid concurrent use.
Warfarin/Anticoagulants
Increased risk of bleeding.
Drugs metabolized by CYP2D6 (e.g., TCAs, Flecainide)
Fluoxetine can inhibit CYP2D6, increasing levels of co-administered drugs.
Other Serotonergic Drugs (e.g., Triptans, Tramadol, other SSRIs)
Increased risk of serotonin syndrome.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, seizures, cardiovascular dysfunction, and serotonin syndrome. Treatment is symptomatic and supportive; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Fluoxetine is excreted in breast milk; avoid use during breastfeeding or monitor infant for adverse effects.
