Description
Overview of the medicine
Dermasol-N 0.05% Ointment is a combination medication containing a potent corticosteroid (Clobetasol Propionate) and an antibiotic (Neomycin Sulfate). It is used to treat inflammatory skin conditions that are complicated by bacterial infections.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily, as directed by a physician. Treatment should be limited to 1-2 weeks.
Elderly
No specific dosage adjustment is usually required, but caution should be exercised due to potentially thinner skin and increased susceptibility to side effects.
Renal_impairment
No specific dosage adjustment for topical application. However, caution with long-term or extensive use in severe renal impairment due to potential systemic absorption of Neomycin.
How to Take
For external use only. Apply a small amount to the affected skin area and rub in gently. Wash hands thoroughly before and after application.
Mechanism of Action
Clobetasol Propionate works as an anti-inflammatory, immunosuppressive, and vasoconstrictive agent. Neomycin Sulfate inhibits bacterial protein synthesis, leading to its bactericidal effect.
Pharmacokinetics
Onset
Onset of action is typically within a few days of regular application.
Excretion
Clobetasol and its metabolites are primarily excreted by the kidneys. Absorbed Neomycin is also excreted primarily via the kidneys.
Half life
Pharmacokinetic data specifically for topical combination are not extensively documented. Systemic Clobetasol has a half-life of about 4-5 hours. Systemic Neomycin has a half-life of 2-3 hours.
Absorption
Topical absorption varies depending on skin integrity, application site, and use of occlusive dressings. Systemic absorption of Clobetasol is minimal but can occur, leading to hypothalamic-pituitary-adrenal (HPA) axis suppression. Neomycin can also be absorbed systemically.
Metabolism
Clobetasol is primarily metabolized in the liver. Neomycin is poorly metabolized.
Side Effects
Contraindications
- Viral skin infections (e.g., herpes simplex, varicella)
- Fungal skin infections (unless concurrent antifungal therapy is used)
- Tuberculosis of the skin
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Known hypersensitivity to Clobetasol, Neomycin, or any other component of the ointment
Drug Interactions
Systemic Aminoglycosides
Although systemic absorption of topical Neomycin is minimal, concurrent use of other systemic aminoglycosides should be approached with caution due to potential additive toxicity (e.g., ototoxicity, nephrotoxicity).
Other topical corticosteroids
Concurrent use may increase systemic absorption and potential for adverse effects.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute systemic effects. However, prolonged or excessive use may lead to systemic effects such as HPA axis suppression, Cushing's syndrome, and local adverse effects (e.g., skin atrophy, striae). Systemic absorption of Neomycin can potentially lead to ototoxicity or nephrotoxicity, especially with damaged skin or prolonged use.
Pregnancy & Lactation
Use during pregnancy and lactation only if the potential benefit justifies the potential risk to the fetus or infant. Consult a doctor. Generally, potent topical corticosteroids should be used sparingly and for the shortest duration possible.

Clinical Trials
As a well-established combination, new clinical trials for its basic indications are rare. Its efficacy and safety are supported by numerous studies on its individual components and historical clinical use.
Lab Monitoring
- Not routinely required for typical topical use. For prolonged or extensive use, HPA axis suppression may be monitored (e.g., plasma cortisol levels).
Doctor Notes
- Emphasize short-term use and appropriate indications.
- Advise patients about potential local side effects, especially with prolonged use.
- Caution against use on the face, in intertriginous areas, or in children without specific guidance.
Patient Guidelines
- Use only as directed by your doctor.
- Apply a thin layer to the affected area.
- Do not use on the face, groin, or axillae, or for diaper dermatitis unless specifically instructed by your doctor.
- Avoid contact with eyes and mucous membranes.
- Do not use for longer than the prescribed duration (usually 1-2 weeks).
- Do not cover the treated area with occlusive dressings unless advised by your doctor.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular schedule. Do not apply a double dose.
Driving Precautions
Dermasol-N Ointment is for topical use and is not expected to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain good hygiene in the affected area.
- Avoid irritants or allergens that may worsen the skin condition.
