Description
Overview of the medicine
Dersole-005 Ointment is a potent topical corticosteroid containing Clobetasol Propionate 0.05%. It is used for the short-term treatment of inflammatory skin conditions such as severe eczema, psoriasis, and other dermatoses that respond to steroids, when less potent corticosteroids have been ineffective.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than 4 consecutive weeks. Treatment should be limited to short courses and small areas.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential for increased systemic absorption and thinner skin.
Renal_impairment
No specific dosage adjustment is generally required for topical use, but systemic absorption should be monitored if used extensively.
How to Take
Apply a small amount of ointment to the affected skin area and gently rub it in until it disappears. Wash hands immediately after application, unless hands are the treated area. Do not use occlusive dressings unless specifically advised by a doctor.
Mechanism of Action
Clobetasol Propionate is a highly potent corticosteroid that exhibits anti-inflammatory, antipruritic, and vasoconstrictive actions. It acts by inducing phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Symptomatic relief can occur within a few days of consistent application.
Excretion
Excreted primarily by the kidneys, and to a lesser extent, via bile.
Half life
Not well-defined for topical application; depends on systemic absorption and metabolism.
Absorption
Systemic absorption of topical corticosteroids can occur, especially through compromised skin, prolonged use, or with occlusive dressings. Absorption is higher in areas with thin skin, like the face and genitals.
Metabolism
Once absorbed, clobetasol propionate is primarily metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any other component of the preparation
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Pruritus without inflammation
- Perianal and genital pruritus
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox)
- Fungal infections (e.g., candidiasis, tinea)
- Bacterial infections (e.g., impetigo)
- Children under 12 years of age for long-term use
Drug Interactions
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol, leading to an increased risk of systemic side effects. Concomitant use should be undertaken with caution.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with topical corticosteroids is unlikely. In the event of chronic overdose or misuse, symptoms of hypercorticism (e.g., Cushing's syndrome) may appear due to systemic absorption. Treatment should be gradual withdrawal of the steroid, under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether topical administration of corticosteroids can result in sufficient systemic absorption to produce detectable quantities in breast milk. Use with caution in nursing mothers.

Clobetasol Propionate has been extensively studied in numerous clinical trials demonstrating its efficacy and safety for various steroid-responsive dermatoses.
Lab Monitoring
- Adrenal function tests (e.g., plasma cortisol levels) may be considered if extensive use or HPA axis suppression is suspected, especially in pediatric patients.
Doctor Notes
- Emphasize short-term use (max 4 weeks) and avoidance on sensitive areas like the face.
- Educate patients on potential for systemic absorption and adrenal suppression, especially in pediatric patients.
- Monitor for signs of local skin reactions and systemic side effects.
Patient Guidelines
- Apply sparingly and thinly to the affected area.
- Do not use for longer than the prescribed period (usually 2-4 weeks).
- Avoid application to the face, groin, or axillae unless specifically directed by a doctor.
- Wash hands after application.
