Description
Overview of the medicine
Dexlazo (Dexlansoprazole) is a proton pump inhibitor (PPI) used to treat symptoms of gastroesophageal reflux disease (GERD), heal erosive esophagitis (EE), and maintain healing of EE.
Uses & Indications
Dosage
Adults
For healing EE: 60 mg once daily for up to 8 weeks. For maintenance of healed EE and relief of heartburn: 30 mg once daily. For symptomatic GERD: 30 mg once daily for 4 weeks.
Elderly
No dosage adjustment is generally required. However, monitor for potential adverse effects.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take orally, with or without food. Swallow the capsule whole; do not chew, crush, or open the capsule.
Mechanism of Action
Dexlansoprazole suppresses gastric acid secretion by irreversibly inhibiting the H+/K+-ATPase enzyme system at the secretory surface of the gastric parietal cell, thereby blocking the final step of acid production.

Excretion
Primarily excreted via urine (approximately 50%) and feces (approximately 23%) as inactive metabolites.
Half life
Approximately 1-2 hours (terminal elimination half-life)
Absorption
Rapidly absorbed after oral administration with two distinct peaks due to the dual delayed release formulation. Absolute bioavailability is approximately 75%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 (CYP) enzymes, primarily CYP2C19 and CYP3A4, to inactive hydroxylated and sulfated metabolites.
Side Effects
Contraindications
- Known hypersensitivity to dexlansoprazole or any component of the formulation.
- Coadministration with products containing rilpivirine.
Drug Interactions
Digoxin
Increased digoxin absorption and levels.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Iron Salts
Decreased absorption of iron salts.
Clopidogrel
Reduced effectiveness of clopidogrel due to CYP2C19 inhibition.
Methotrexate
Possible increased methotrexate levels, especially with high doses.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Management should be symptomatic and supportive. Dexlansoprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Excretion into breast milk is unknown; caution should be exercised when administered to a nursing woman.

- Eat smaller, more frequent meals instead of large ones.
- Avoid lying down within 3 hours after eating.
- Elevate the head of your bed by 6-8 inches.
