Description
Overview of the medicine
Diampa 25 mg Tablet contains Dapagliflozin, an oral antidiabetic drug used to improve glycemic control in adults with type 2 diabetes mellitus. It also reduces the risk of cardiovascular death and hospitalization for heart failure in adults with heart failure, and slows the progression of kidney disease in adults with chronic kidney disease.
Uses & Indications
Dosage
Adults
The typical recommended dose of Dapagliflozin is 10 mg once daily. For a 25 mg tablet, dosage should be strictly as prescribed by a physician, considering that 25 mg is a higher than usual dose for Dapagliflozin.
Elderly
No dose adjustment is generally required based on age alone. However, renal function should be monitored due to increased risk of dehydration and potential for renal impairment.
Renal_impairment
Initiation is not recommended in patients with eGFR < 25 mL/min/1.73 m². Dose adjustment may be needed based on eGFR. Consult physician.
How to Take
Take orally once daily, with or without food, preferably in the morning.
Mechanism of Action
Dapagliflozin inhibits Sodium-Glucose Co-Transporter 2 (SGLT2) in the renal proximal tubules, reducing glucose reabsorption and increasing urinary glucose excretion, thereby lowering blood glucose levels. It also promotes osmotic diuresis and natriuresis.
Pharmacokinetics
Onset
Within 30-60 minutes for glucose lowering effects.
Excretion
Excreted via urine (approx. 75%) and feces (approx. 21%), mainly as inactive metabolites.
Half life
Approximately 12.9 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 2 hours. Absolute bioavailability is approximately 78%.
Metabolism
Primarily metabolized by UGT1A9 to an inactive metabolite (dapagliflozin 3-O-glucuronide).
Side Effects
Contraindications
- Hypersensitivity to dapagliflozin or any component of the formulation.
- Patients on dialysis.
- Diabetic ketoacidosis (DKA).
Drug Interactions
ACE Inhibitors / ARBs
Potential for increased risk of hypotension, especially in volume-depleted patients.
Diuretics (e.g., Thiazide, Loop diuretics)
May increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues (e.g., Sulfonylureas)
May increase the risk of hypoglycemia. A lower dose of insulin or secretagogue may be required.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to dehydration, hypotension, and electrolyte imbalance. Management should involve supportive care, monitoring of hydration status and electrolytes, and symptomatic treatment.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy due to potential for adverse renal development. Avoid during lactation as it may be excreted in breast milk.

WHO Essential Medicine
YesClinical Trials
Dapagliflozin has been evaluated in extensive clinical trials such as DECLARE-TIMI 58 (cardiovascular outcomes in T2DM), DAPA-HF (heart failure with reduced ejection fraction), and DAPA-CKD (chronic kidney disease), demonstrating significant benefits.
Lab Monitoring
- Renal function (eGFR) before initiation and periodically thereafter.
- Glycemic control (HbA1c, fasting glucose).
- Electrolytes, especially in patients at risk of volume depletion.
Doctor Notes
- Emphasize patient education on proper hydration, self-monitoring for symptoms of DKA, and genital/urinary infections.
- Monitor renal function (eGFR) and volume status regularly, especially in vulnerable populations.
- Consider dose adjustments for concomitant use with diuretics or insulin/sulfonylureas.
Patient Guidelines
- Drink plenty of fluids to avoid dehydration, especially during illness or in hot weather.
- Monitor for symptoms of urinary tract infections or genital mycotic infections (e.g., itching, discharge, burning).
- Seek immediate medical attention if symptoms of ketoacidosis (e.g., nausea, vomiting, abdominal pain, difficulty breathing, unusual fatigue) occur.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
May cause dizziness; caution is advised, especially when initiating therapy or if taken with other medications that can lower blood glucose (e.g., insulin, sulfonylureas).
Lifestyle Advice
- Maintain a balanced diet and regular exercise regimen as advised by your healthcare provider.
- Regularly monitor blood glucose levels and adhere to medication schedule.
