Description
Overview of the medicine
Dianide 500 mg Tablet contains Metformin Hydrochloride, an oral antidiabetic drug used to manage type 2 diabetes mellitus. It helps to lower blood glucose levels by decreasing glucose production in the liver and improving insulin sensitivity.
Uses & Indications
Dosage
Adults
Initial dose 500 mg once or twice daily with meals. Max dose 2000-2550 mg/day.
Elderly
Dose adjustment may be necessary due to potential decreased renal function. Renal function should be monitored regularly.
Renal_impairment
Contraindicated if eGFR < 30 mL/min/1.73 m². Dose reduction if eGFR 30-45 mL/min/1.73 m².
How to Take
Dianide 500 mg Tablet is taken orally, usually with meals, to reduce gastrointestinal side effects.
Mechanism of Action
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Pharmacokinetics
Onset
Within days of starting therapy, full effect in 2 weeks.
Excretion
Excreted unchanged in the urine.
Half life
Approximately 6.2 hours.
Absorption
Bioavailability is 50-60%. Food decreases absorption. Peak plasma concentration is reached in 2-3 hours.
Metabolism
Not metabolized.
Side Effects
Contraindications
- Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
- Acute or chronic metabolic acidosis, including diabetic ketoacidosis
- Hypersensitivity to metformin
Drug Interactions
Iodinated Contrast Agents
Increased risk of lactic acidosis. Temporarily discontinue before or at the time of the procedure.
Carbonic Anhydrase Inhibitors (e.g., topiramate)
May increase the risk of lactic acidosis.
Cationic Drugs (e.g., amiloride, digoxin, morphine, ranitidine, triamterene)
May compete for renal tubular transport, increasing metformin concentration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of Metformin can lead to lactic acidosis. Management includes hemodialysis to remove metformin, supportive treatment, and correction of acidosis.
Pregnancy & Lactation
Pregnancy: Generally, insulin is preferred for blood glucose control during pregnancy. Metformin may be considered in some cases under strict medical supervision. Lactation: Metformin is excreted into breast milk. Benefits of breastfeeding versus risk to infant should be considered.

Lab Monitoring
- Renal function (eGFR) before initiation and at least annually
- HbA1c (every 3-6 months)
- Vitamin B12 levels (for long-term use, especially if symptoms of deficiency appear)
Doctor Notes
- Emphasize patient education on lactic acidosis symptoms and when to seek emergency care.
- Advise regular monitoring of renal function and vitamin B12 levels, especially in elderly patients or those on long-term therapy.
Patient Guidelines
- Take Dianide 500 mg Tablet with meals to minimize gastrointestinal upset.
- Do not crush or chew extended-release tablets.
- Inform your doctor if you have kidney problems, are pregnant, or breastfeeding.
- Report any symptoms of lactic acidosis (e.g., unusual muscle pain, difficulty breathing, unusual stomach discomfort, dizziness, lightheadedness, slow or irregular heartbeat).
Missed Dose Advice
If a dose is missed, take it as soon as remembered with food. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Generally does not cause hypoglycemia when used alone, thus unlikely to impair driving ability. However, if used in combination with other antidiabetic agents that can cause hypoglycemia, caution is advised.
Lifestyle Advice
- Maintain a healthy diet and regular exercise routine.
- Monitor blood glucose levels regularly.
- Limit alcohol consumption.
