Diapiotab 15 mg Tablet contains Pioglitazone, an oral antidiabetic medicine used to treat type 2 diabetes mellitus. It helps to control blood sugar levels by increasing the body's sensitivity to insulin.
Uses & Indications
Dosage
Adults
Initial dose is 15 mg or 30 mg once daily. The dose can be increased up to a maximum of 45 mg once daily, based on glycemic response. The 15 mg tablet is suitable for starting dose or maintenance.
Elderly
No specific dose adjustment is required, but caution is advised due to potential for increased risk of adverse effects.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Pioglitazone is a potent and highly selective agonist for peroxisome proliferator-activated receptor-gamma (PPAR-gamma). PPAR-gamma receptors are found in adipose tissue, skeletal muscle, and the liver. Activation of PPAR-gamma receptors modulates the transcription of insulin-responsive genes involved in the control of glucose and lipid metabolism, leading to increased insulin sensitivity in target tissues.

The full therapeutic effect is typically observed within 2-3 weeks, with maximum effect potentially taking up to 2-3 months.
Excretion
Approximately 15-30% of metabolites excreted in urine and the remainder via feces.

Parent drug: 3-7 hours; active metabolites: 16-24 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 isoenzymes, primarily CYP2C8 and CYP3A4, to active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to pioglitazone or any component of the formulation.
- Patients with active heart failure (NYHA Class III or IV).
- Severe hepatic impairment.
- History of bladder cancer.
- Diabetic ketoacidosis (DKA).
Drug Interactions
Insulin/Sulfonylureas
Increased risk of hypoglycemia when used in combination; dose adjustments may be needed.
Rifampicin (CYP2C8 inducer)
Coadministration with rifampicin significantly decreases pioglitazone exposure; consider increasing the dose of pioglitazone.
Gemfibrozil (CYP2C8 inhibitor)
Coadministration with gemfibrozil significantly increases pioglitazone exposure; consider a reduced dose of pioglitazone.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of an overdose, symptomatic and supportive measures should be taken. Gastric lavage and activated charcoal may be considered. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during lactation as it is unknown if pioglitazone is excreted in human milk.
