Description
Overview of the medicine
Diapizi 25 mg Tablet contains Pizirapam Hydrochloride, a hypothetical selective serotonin reuptake enhancer (SSRE) indicated for the treatment of major depressive disorder and generalized anxiety disorder. It helps in restoring the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Initial dose 25 mg once daily, preferably in the evening. May be increased to 50 mg daily after 1 week based on response and tolerability. Max dose 75 mg daily.
Elderly
Initial dose 12.5 mg once daily. Max 50 mg daily. Dose adjustments should be made carefully due to increased sensitivity and potential for adverse effects.
Renal_impairment
Mild to moderate impairment: No initial dose adjustment. Severe impairment: Initial dose 12.5 mg once daily. Use with caution.
How to Take
Diapizi Tablet should be swallowed whole with water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Pizirapam is hypothesized to selectively enhance the reuptake of serotonin from the synaptic cleft, potentially leading to improved neurotransmission in mood and anxiety-related pathways. Unlike SSRIs, it acts by enhancing rather than inhibiting reuptake, resulting in distinct pharmacological effects.
Pharmacokinetics
Onset
Initial therapeutic effects may be observed within 1-2 weeks, with full effects typically seen after 4-6 weeks of consistent use.
Excretion
Excreted mainly through the kidneys as inactive metabolites, with a small percentage excreted in feces.
Half life
Approximately 12-18 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations achieved within 1-2 hours.
Metabolism
Primarily metabolized in the liver via CYP450 enzymes (e.g., CYP2D6 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Pizirapam or any excipients of the tablet.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy.
Drug Interactions
MAOIs
Concurrent use is contraindicated due to risk of serotonin syndrome.
Alcohol
May potentiate central nervous system depressant effects; avoid concomitant use.
Warfarin
May enhance anticoagulant effects; monitor INR closely.
SSRIs/SNRIs
Increased risk of serotonin syndrome, use with caution.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe dizziness, drowsiness, confusion, rapid heart rate, tremors, seizures, and possibly coma. Treatment is symptomatic and supportive, including maintaining an open airway, monitoring cardiac and vital signs, and gastric lavage if ingested recently. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pizirapam is likely excreted in human milk; use with caution in breastfeeding mothers, weighing benefits against risks.
