Gliclazide is an oral antidiabetic drug used to manage type 2 diabetes mellitus in adults. It belongs to the sulfonylurea class and works by stimulating insulin release from pancreatic beta cells. The modified-release (MR) formulation allows for once-daily dosing and provides prolonged glycemic control.

Dosage
Adults
The usual starting dose is 30 mg once daily, taken with breakfast. The dose can be adjusted in increments of 30 mg, up to a maximum of 120 mg once daily, based on blood glucose response.
Elderly
No specific dose adjustment is usually required, but careful monitoring of blood glucose is advised due to increased risk of hypoglycemia.
Renal_impairment
In mild to moderate renal impairment (creatinine clearance 30-60 mL/min), the standard dosing regimen may be used with careful monitoring. Avoid in severe renal impairment.
How to Take
Diazid MR tablet should be taken orally once daily with breakfast. The tablet must be swallowed whole with a glass of water, not chewed, crushed, or divided.
Mechanism of Action
Gliclazide acts by stimulating insulin secretion from pancreatic beta cells. It binds to the sulfonylurea receptor (SUR1) on the beta-cell membrane, leading to the closure of ATP-sensitive potassium channels. This depolarization opens voltage-dependent calcium channels, increasing intracellular calcium, which triggers insulin exocytosis.

Excretion
Metabolites are primarily excreted in the urine (about 60-70%) and feces (10-20%).

The apparent plasma half-life of gliclazide MR is approximately 12-20 hours, allowing for once-daily administration.
Absorption
Well absorbed from the gastrointestinal tract. The modified release formulation results in a gradual and sustained absorption. Peak plasma concentrations are typically reached within 6-12 hours after administration.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme CYP2C9, into inactive metabolites. Less than 1% is excreted unchanged in the urine.
Side Effects
Contraindications
- Hypersensitivity to gliclazide, other sulfonylureas, or any excipients.
- Type 1 Diabetes Mellitus.
- Diabetic ketoacidosis, diabetic precoma and coma.
- Severe renal or hepatic insufficiency.
- Concomitant use with miconazole (oral, systemic gel).
- Pregnancy and lactation.

Anticoagulants (e.g., Warfarin)
Gliclazide may potentiate the effect of anticoagulants, requiring dose adjustment and monitoring.
Miconazole (systemic, oral gel)
Potentiates the hypoglycemic effect with possible onset of hypoglycemia, even coma.
Phenylbutazone, Alcohol, other antidiabetics
Increased risk of hypoglycemia.
Corticosteroids, Danazol, Chlorpromazine, Ritodrine, Salbutamol (IV), Diuretics
May decrease the hypoglycemic effect, leading to hyperglycemia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach and sight of children.
Overdose
Overdose may lead to severe hypoglycemia. Symptoms include sweating, pallor, hunger, tremors, confusion, headache, and in severe cases, seizures or coma. Management involves immediate administration of oral glucose (for conscious patients) or intravenous glucose/glucagon (for unconscious patients).
Pregnancy & Lactation
Gliclazide is contraindicated during pregnancy. Insulin is the preferred treatment for diabetes in pregnant women. It is also contraindicated during lactation as sulfonylureas may be excreted in breast milk.
