Description
Overview of the medicine
Doxofylline is a bronchodilator used to treat bronchial asthma and chronic obstructive pulmonary disease (COPD). It helps to relax the muscles in the airways, making breathing easier.
Uses & Indications
Dosage
Adults
400 mg orally, 2-3 times daily, adjusted according to clinical response and tolerability. Maximum dose usually 1200 mg/day.
Elderly
Similar to adults, but caution is advised due to potential for altered pharmacokinetics; lower doses may be considered.
Renal_impairment
Use with caution. Dose adjustment may be necessary in severe renal impairment.
How to Take
Doxofylline tablets should be taken orally, with or without food. Swallow the tablet whole with water; do not crush or chew.
Mechanism of Action
Doxofylline acts as a phosphodiesterase inhibitor, leading to an increase in intracellular cyclic AMP (cAMP) levels. This results in bronchodilation and also exhibits anti-inflammatory effects. Unlike theophylline, it has a better safety profile and fewer drug interactions.
Pharmacokinetics
Onset
Bronchodilatory effects typically begin within 30-60 minutes.
Excretion
Mainly excreted in the urine as metabolites.
Half life
Approximately 6-10 hours, varying among individuals.
Absorption
Rapidly absorbed from the gastrointestinal tract after oral administration.
Metabolism
Extensively metabolized in the liver (primarily via CYP450 system) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to doxofylline or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- Severe cardiac arrhythmias
Drug Interactions
Cimetidine
May increase plasma concentrations of doxofylline by inhibiting its metabolism.
Erythromycin
Can increase doxofylline levels, requiring dose adjustment.
Phenobarbital/Phenytoin
May decrease doxofylline efficacy by inducing its metabolism.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, epigastric pain, headache, tachycardia, ventricular arrhythmias, and convulsions. Management is symptomatic and supportive, including gastric lavage and administration of activated charcoal. Close cardiac monitoring is essential.
Pregnancy & Lactation
Pregnancy Category C. Use with caution during pregnancy; weigh potential benefits against risks. Doxofylline is excreted in breast milk, so caution is advised for nursing mothers.

Clinical Trials
Several clinical trials have demonstrated doxofylline's efficacy and safety in managing asthma and COPD, often showing a better tolerability profile compared to theophylline.
Lab Monitoring
- Routine monitoring of doxofylline plasma levels is generally not required, unlike theophylline. However, clinical monitoring for efficacy and side effects is important.
Doctor Notes
- Consider dose reduction in patients with hepatic impairment.
- Monitor for gastrointestinal disturbances and central nervous system effects.
- Advise patients to avoid abrupt discontinuation without medical consultation.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not exceed the recommended dose.
- Inform your doctor about any other medications you are taking.
- Report any unusual or severe side effects immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Doxofylline may cause dizziness or blurred vision in some individuals. Patients should be cautious when driving or operating machinery until they know how the medicine affects them.
Lifestyle Advice
- Avoid smoking, as it can reduce the effectiveness of doxofylline.
- Limit caffeine intake to minimize potential side effects.
- Maintain a healthy lifestyle to support respiratory health.
