Description
Overview of the medicine
Elena 40 mg capsule contains Esomeprazole, a proton pump inhibitor that reduces the amount of acid produced in the stomach. It is used to treat conditions like GERD, stomach ulcers, and to eradicate H. pylori.
Uses & Indications
Dosage
Adults
For Erosive Esophagitis: 40 mg once daily for 4 to 8 weeks. For Symptomatic GERD: 20 mg once daily for 4 weeks. For H. pylori eradication: 40 mg once daily in combination with antibiotics for 10 days. For NSAID-associated ulcers: 20 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: 40 mg twice daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but caution should be exercised in patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is required in patients with renal impairment.
How to Take
Take one capsule orally at least one hour before a meal. Swallow the capsule whole with water. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. This action effectively inhibits the final step in acid production, leading to a sustained reduction in gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour.
Excretion
Approximately 80% is excreted in the urine as metabolites, and the remainder in the feces.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose, increasing to 89% after repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Esomeprazole, substituted benzimidazoles, or any excipients in the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel

Methotrexate
Increased and prolonged elevated serum levels of methotrexate and/or its metabolite.
Atazanavir, Nelfinavir
Reduced plasma levels of these antiretrovirals (contraindicated with nelfinavir, avoid with atazanavir).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms are generally mild and reversible. No specific antidote is known. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is likely excreted in human milk; caution should be exercised when administered to a nursing woman.
