Description
Overview of the medicine
Emidom 5 mg Suspension contains Domperidone, an antiemetic and prokinetic agent. It is used to treat nausea, vomiting, indigestion, and stomach discomfort by blocking dopamine receptors in the stomach and brain.
Uses & Indications
Dosage
Adults
10-20 mL (10-20 mg) 3-4 times daily, 15-30 minutes before meals and at bedtime. Maximum 80 mg/day.
Elderly
Lower doses may be required due to potential for prolonged QT interval, especially in those with renal or hepatic impairment. Close monitoring is advised.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), frequency should be reduced to 1-2 times daily depending on severity. Consult a physician.
How to Take
Administer orally, preferably 15-30 minutes before meals. Use a measuring cup or syringe for accurate dosing.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2 receptor antagonist, primarily at the chemoreceptor trigger zone (CTZ) and in the gastrointestinal tract. This increases esophageal and gastric peristalsis, lower esophageal sphincter pressure, and accelerates gastric emptying.

Side Effects
Contraindications
- Known hypersensitivity to domperidone or any component of the formulation
- Prolactin-releasing pituitary tumor (prolactinoma)
- Moderate to severe hepatic impairment
- Conditions where gastrointestinal stimulation is dangerous (e.g., GI hemorrhage, mechanical obstruction, perforation)
- Concomitant use of potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
- Patients with known prolonged QT intervals or significant cardiac disease (e.g., CHF)
Drug Interactions
Anticholinergics
May antagonize the prokinetic effect of domperidone.
QT-prolonging drugs
Increased risk of arrhythmias when used with other drugs that prolong the QT interval (e.g., amiodarone, cisapride, sotalol, some antipsychotics).
Dopaminergic agonists
Domperidone may antagonize the peripheral effects of dopaminergic agonists like bromocriptine.
Potent CYP3A4 inhibitors

Overdose
Symptoms of overdose include drowsiness, disorientation, and extrapyramidal reactions (especially in children). Management involves gastric lavage, activated charcoal, symptomatic and supportive treatment. Anticholinergics or antiparkinsonian agents may be used for extrapyramidal symptoms.
