Description
Overview of the medicine
Emo-20 contains Escitalopram, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder and generalized anxiety disorder. It works by restoring the balance of a natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose 10 mg once daily, may be increased to 20 mg once daily after at least one week. Maximum dose 20 mg/day.
Elderly
Recommended initial dose 5 mg once daily. Maximum recommended dose 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
Take Emo-20 orally once daily, with or without food. It is generally recommended to take it in the morning to avoid potential insomnia, but can be taken at any time of day.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) by presynaptic neurons, leading to increased serotonin concentration in the synaptic cleft. This potentiation of serotonergic neurotransmission is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial antidepressant effects may be seen within 2-4 weeks, full effects may take 8-12 weeks.
Excretion
Mainly excreted in urine (approximately 10%) and feces (approximately 27%) as metabolites.
Half life
Approximately 27-32 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, independent of food. Peak plasma concentrations are reached in approximately 5 hours.
Metabolism
Primarily metabolized in the liver by CYP2C19 and CYP3A4 to inactive or weakly active metabolites.
Side Effects
Contraindications

MAOIs
Concurrent use is contraindicated due to risk of serotonin syndrome.
Pimozide
Concurrent use is contraindicated due to risk of QT prolongation.
Warfarin
Increased risk of bleeding; monitor INR.
NSAIDs/Aspirin
Increased risk of bleeding.
Lithium/Tryptophan
May increase serotonergic effects.

Lab Monitoring
- No specific routine lab monitoring is generally required. However, consider baseline and periodic monitoring of electrolytes (especially sodium) in patients at risk for hyponatremia. Liver and renal function tests may be considered in patients with pre-existing impairment.
Doctor Notes
- Advise patients that full therapeutic effects may take several weeks.
- Monitor for worsening depression, suicidality, or unusual behavioral changes, especially at the start of treatment or dose changes.
- Educate patients about potential withdrawal symptoms and the importance of gradual dose reduction.
