Description
Overview of the medicine
Escitalopram is an antidepressant used to treat major depressive disorder and generalized anxiety disorder. It works by increasing serotonin levels in the brain.
Uses & Indications
Dosage
Adults
Initial 5 mg once daily, usually for a week before increasing to 10 mg. Maximum 20 mg daily.
Elderly
Initial 5 mg once daily, maximum 10 mg daily.
Renal_impairment
No dosage adjustment usually required for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Orally, once daily, with or without food. Take at the same time each day.
Mechanism of Action
Selectively inhibits the reuptake of serotonin (5-HT) in the brain, increasing serotonin concentration in the synaptic cleft.
Pharmacokinetics
Onset
Clinical effects may take 2-4 weeks to become apparent.
Excretion
Primarily via urine, with some fecal excretion.
Half life
Approximately 27-32 hours.
Absorption
Well absorbed orally, unaffected by food. Peak plasma concentration reached in about 4 hours.
Metabolism
Primarily hepatic, via CYP2C19, CYP3A4, and CYP2D6 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to escitalopram or citalopram.
- Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing an MAOI.
- Concomitant use with pimozide.
Drug Interactions

Increased risk of serotonin syndrome.
Alcohol
Avoid concomitant use.
Pimozide
Increased QT prolongation.
Warfarin, NSAIDs
Increased risk of bleeding.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Dizziness, tremor, nausea, vomiting, somnolence, tachycardia, hypotension, rarely QT prolongation, arrhythmias, convulsions, serotonin syndrome. Management: Supportive and symptomatic treatment. Maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered. No specific antidote.
Pregnancy & Lactation
Pregnancy: Category C. Use only if potential benefit outweighs risk. Neonatal withdrawal symptoms have been reported. Lactation: Excreted in breast milk. Use with caution, consult doctor.
