Overview of the medicine
Erazole 40 mg Capsule contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in your stomach. It is used to treat acid-related diseases of the stomach and gut.
Uses & Indications
Dosage
Adults
For erosive esophagitis: 20 mg or 40 mg once daily for 4 to 8 weeks. For maintenance of healing: 20 mg once daily. For symptomatic GERD: 20 mg once daily for 4 weeks. For H. pylori eradication: 20 mg twice daily for 10 days (in combination with antibiotics). For Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment is generally needed, but monitor patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Erazole capsules should be swallowed whole with water. Do not chew or crush the capsules. It is recommended to take the capsule at least one hour before a meal.
Mechanism of Action
Esomeprazole is a specific inhibitor of the H+/K+-ATPase (proton pump) in the gastric parietal cell. It inhibits both basal and stimulated acid secretion, leading to a profound and prolonged reduction in gastric acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression occurs within one hour of oral administration.
Excretion
Approximately 80% of the dose is excreted as metabolites in the urine, and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose and increases to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral).
Drug Interactions
Warfarin
Increased INR and prothrombin time, potentially leading to abnormal bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition.
Methotrexate
Increased and prolonged elevation of methotrexate and/or its metabolite serum levels.
Nelfinavir/Atazanavir
Decreased plasma concentrations of these antiretrovirals, reducing their efficacy.
Diazepam, Citalopram, Imipramine, Phenytoin
Increased plasma levels of these drugs due to inhibition of CYP2C19.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
There is limited experience with esomeprazole overdose. Symptoms are generally mild and reversible. Management should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in breast milk; caution should be exercised when administered to a nursing woman.
