Esomeprazole is a proton pump inhibitor that reduces gastric acid secretion. It is used to treat conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and Zollinger-Ellison syndrome.

Gastroesophageal Reflux Disease (GERD)

Healing of erosive esophagitis

Maintenance of healing of erosive esophagitis

Symptomatic GERD

Risk reduction of NSAID-associated gastric ulcers

H. pylori eradication (in combination with antibiotics)

Zollinger-Ellison Syndrome

Dosage

Adults

For Erosive Esophagitis: 20 or 40 mg once daily for 4 to 8 weeks. For GERD symptoms: 20 mg once daily for 4 weeks. H. pylori eradication: 20 mg twice daily for 10 days, with appropriate antibiotics.

Elderly

No specific dosage adjustment is necessary based on age alone.

Renal_impairment

No dosage adjustment is generally required for patients with renal impairment.

How to Take

Take orally, usually once daily, at least one hour before a meal. Swallow the capsule whole; do not chew or crush.

Mechanism of Action

Esomeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion.

Absorption

Rapidly absorbed after oral administration. Bioavailability is about 64% for a single 40 mg dose and increases to 89% with repeated once-daily administration. Food intake may delay and decrease absorption.

Metabolism

Extensively metabolized in the liver by the cytochrome P450 system, mainly by CYP2C19 and to a lesser extent by CYP3A4.

Side Effects

Contraindications

Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.

Drug Interactions

Warfarin

May increase INR and prothrombin time, monitor carefully.

Clopidogrel

May reduce the antiplatelet effect of clopidogrel. Avoid concomitant use.

Methotrexate

May increase methotrexate levels, especially with high doses.

Atazanavir/Nelfinavir

May decrease plasma concentrations of these antivirals, reducing their efficacy.

Storage

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Overdose

Limited experience. Symptoms might include drowsiness, blurred vision, tachycardia, nausea, vomiting, sweating, headache, dry mouth. Treatment is symptomatic and supportive.

Pregnancy & Lactation

Pregnancy Category B. Use only if clearly needed. Excretion into breast milk is unknown, use with caution in nursing mothers.
