Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, and ulcers.
Uses & Indications
Dosage
Adults
For erosive esophagitis: 20 mg or 40 mg once daily for 4 to 8 weeks. For GERD symptoms: 20 mg once daily for 4 weeks. For H. pylori eradication: 40 mg once daily with amoxicillin and clarithromycin for 10 days.
Elderly
No specific dosage adjustment required; however, caution should be exercised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment needed for renal impairment.
How to Take
Take orally, at least one hour before a meal. Swallow the tablet whole; do not chew or crush. For patients who have difficulty swallowing, the tablet can be dispersed in a glass of water.
Mechanism of Action
Esomeprazole is an irreversible proton pump inhibitor. It binds to and inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby blocking the final step in gastric acid secretion.

Mainly excreted via urine (approximately 80%) and feces (approximately 20%).

Rapidly absorbed after oral administration, bioavailability is approximately 64% at 40 mg dose, increasing with repeated dosing.

Extensively metabolized in the liver via CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an HIV protease inhibitor).
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to inhibition of CYP2C19.
Methotrexate
Increased plasma levels of methotrexate, particularly at high doses.
Atazanavir, Nelfinavir
Reduced plasma levels of antiretrovirals; avoid co-administration with nelfinavir.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Experience with esomeprazole overdose is limited. Symptoms may include gastrointestinal disturbances and weakness. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk, but human studies are insufficient. Use only if clearly needed. Excreted in human milk, use with caution during lactation.
