Esozyme 20 mg tablet contains Esomeprazole, a proton pump inhibitor, used to reduce the amount of acid produced in your stomach. It treats conditions like acid reflux, heartburn, and stomach ulcers.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 10 days (with antibiotics). Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No specific dosage adjustment is required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take Esozyme 20 mg tablet orally, usually once daily, at least 30-60 minutes before a meal. Swallow the tablet whole with water; do not chew or crush it.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase pump (proton pump) in the gastric parietal cells, thereby inhibiting gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour, with maximal acid suppression achieved after several days of dosing.
Excretion
Approximately 80% is excreted in the urine as metabolites, and the remainder is excreted in the feces.
Half life
Plasma elimination half-life is approximately 1-1.5 hours, but the duration of acid suppression is much longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma levels occurring at approximately 1-2 hours. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Digoxin
Increased absorption and plasma concentration of digoxin.
Warfarin
Increased INR and prothrombin time, potentially leading to bleeding.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma concentration of methotrexate, potentially leading to toxicity.
Atazanavir/Nelfinavir
Significant reduction in plasma levels of these antiretrovirals, reducing their efficacy.

Overdose
There is limited experience with overdose. Symptoms may include gastrointestinal disturbances, somnolence, and headache. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother.
