Overview of the medicine
Esypan 5 mg Tablet contains Esomeprazole, a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat acid-related conditions such as gastroesophageal reflux disease (GERD), erosive esophagitis, and gastric ulcers.
Uses & Indications
Dosage
Adults
The 5 mg strength is generally used for specific conditions or as maintenance, often in pediatric patients or for very mild symptoms. Typical adult doses for GERD or ulcers are 20 mg or 40 mg once daily. For specific uses of 5mg, follow physician's guidance.
Elderly
No specific dose adjustment is generally required for elderly patients based on age alone, but caution is advised in very frail or debilitated patients.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take Esypan 5 mg Tablet orally, preferably in the morning at least one hour before a meal. Swallow the tablet whole with water. Do not crush, chew, or break the tablet as this may affect its enteric coating.
Mechanism of Action
Esomeprazole irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. By inhibiting this enzyme, it prevents the final step in gastric acid secretion, leading to a significant and prolonged reduction in stomach acid production.
Pharmacokinetics
Onset
Acid suppression begins within one hour of oral administration.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remaining 20% in the feces.
Half life
Approximately 1.5 hours in healthy subjects, but it has a longer duration of action due to irreversible binding.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations typically achieved within 1 to 2 hours. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and to a lesser extent by CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir (an HIV protease inhibitor).
Drug Interactions
Diazepam
May decrease the clearance of diazepam due to inhibition of CYP2C19.
Warfarin
Increased INR and prothrombin time have been reported, requiring monitoring.
Clopidogrel
Esomeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting its conversion to active metabolite, especially at higher doses (e.g., 40 mg).
Methotrexate
May increase serum methotrexate levels, particularly at high doses of methotrexate, potentially leading to toxicity.
Ketoconazole, Itraconazole
Absorption of these drugs may be decreased due to esomeprazole-induced reduction in intragastric pH.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include gastrointestinal disturbances (e.g., nausea, vomiting, diarrhea) and CNS effects (e.g., headache, lethargy). Management is symptomatic and supportive. Hemodialysis is not effective for removing esomeprazole.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole and its metabolites are excreted in breast milk; caution is advised when administered to a nursing woman.
