Description
Overview of the medicine
Fluvate 50 mcg Nasal Spray contains Fluticasone Propionate, a corticosteroid used to treat and prevent symptoms of allergic rhinitis, including sneezing, runny nose, itchy nose, and watery eyes. It works by reducing inflammation in the nasal passages.
Uses & Indications
Dosage
Adults
For allergic rhinitis: Initially, 2 sprays per nostril once daily. May be reduced to 1 spray per nostril once daily once symptoms are controlled. For nasal polyps: 2 sprays per nostril twice daily.
Elderly
No dosage adjustment required.
Renal_impairment
No dosage adjustment is necessary due to negligible systemic absorption.
How to Take
Administer intranasally. Shake the bottle gently before use. Prime the pump before the first use and if it hasn't been used for several days. Clear nasal passages by gently blowing the nose. Tilt the head slightly forward, insert the nozzle into one nostril, aiming slightly away from the septum. Actuate the spray while sniffing gently. Repeat for the other nostril.
Mechanism of Action
Fluticasone propionate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It binds to glucocorticoid receptors, leading to the inhibition of phospholipase A2, and thereby inhibiting the release of various inflammatory mediators such as histamine, prostaglandins, and leukotrienes. This action reduces inflammation, swelling, and mucus production in the nasal passages.
Pharmacokinetics
Onset
Onset of action typically occurs within several hours, with full therapeutic benefit achieved after 3-4 days of regular use.
Excretion
Mainly excreted in the feces, with approximately 1% excreted in the urine as metabolites.
Half life
The systemic elimination half-life is approximately 8 hours, although local effects in the nasal mucosa are prolonged.
Absorption
Systemic absorption from the nasal route is low, with an average absolute bioavailability of less than 2%.
Metabolism
Extensively metabolized in the liver via cytochrome P450 3A4 (CYP3A4) enzyme, primarily to an inactive carboxylic acid metabolite.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any component of the formulation.
- Untreated localized infection involving the nasal mucosa.
Drug Interactions
Ritonavir
Concurrent use with ritonavir is not recommended due to a significant increase in systemic fluticasone exposure, leading to potential corticosteroid systemic effects.
Other CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole)
Co-administration may increase systemic fluticasone exposure. Use with caution and consider the potential for systemic corticosteroid effects.
Storage
Store at room temperature (15-30°C). Protect from light and freezing. Do not refrigerate.
Overdose
Acute overdose is unlikely due to the low systemic absorption. Chronic overdose may lead to signs and symptoms of hypercorticism, including adrenal suppression. Management involves temporary reduction of dosage or withdrawal of the drug under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if fluticasone propionate is excreted in human milk; use with caution in nursing mothers.
