Description
Overview of the medicine
Formocort 200 mcg Inhalation Capsule is a combination medicine containing Budesonide (an inhaled corticosteroid) and Formoterol Fumarate (a long-acting beta2-agonist). It is used for the regular maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD) in adults and adolescents.
Uses & Indications
Dosage
Adults
Asthma: 1-2 inhalations twice daily. COPD: 1 inhalation twice daily. The dose should be individualized and adjusted to the severity of the disease.
Elderly
No specific dose adjustments are required, but monitor for potential adverse effects.
Renal_impairment
No specific dose adjustment is considered necessary. Use with caution.
How to Take
For inhalation use only with a dry powder inhaler device. Do not swallow the capsules. Patients should be instructed on the correct use of the inhaler and rinse their mouth with water after each dose to minimize the risk of oral candidiasis.
Mechanism of Action
Budesonide is an anti-inflammatory corticosteroid that reduces inflammation in the airways. Formoterol Fumarate is a selective long-acting beta2-adrenergic agonist that relaxes the smooth muscles of the bronchi, leading to bronchodilation and easier breathing.
Pharmacokinetics
Onset
Formoterol: onset of bronchodilation within 1-3 minutes. Budesonide: full therapeutic effects may take several hours to days.
Excretion
Formoterol is excreted primarily via renal elimination. Budesonide metabolites are excreted via renal and fecal routes.
Half life
Formoterol: approximately 10-14 hours. Budesonide: approximately 2-3 hours.
Absorption
Formoterol is rapidly absorbed. Budesonide undergoes significant first-pass metabolism in the liver. Both components have systemic absorption after inhalation.
Metabolism
Both components are primarily metabolized in the liver. Budesonide is extensively metabolized by CYP3A4. Formoterol is primarily metabolized via direct glucuronidation and O-demethylation.
Side Effects
Contraindications
- Hypersensitivity to budesonide, formoterol, or any excipients.
- Acute asthma attacks or status asthmaticus.
- Patients with active pulmonary tuberculosis.
Drug Interactions
Diuretics
May potentiate the hypokalemic effect of beta2-agonists.
Beta-blockers
May reduce or block the effect of formoterol.
Tricyclic antidepressants, MAOIs
May potentiate the effect of formoterol on the cardiovascular system.
CYP3A4 inhibitors (e.g., Ketoconazole, Ritonavir)
May significantly increase plasma levels of budesonide, potentially leading to systemic corticosteroid effects.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose of formoterol may lead to symptoms like tremor, headache, palpitations, and tachycardia. Chronic overdose of budesonide may lead to signs of hypercorticism and adrenal suppression. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Formocort should be used during pregnancy or lactation only if the potential benefit justifies the potential risk to the fetus or infant. Budesonide is considered to have a low risk. Consult a doctor.

YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of budesonide/formoterol combination for the management of asthma and COPD, including studies on different strengths and patient populations.
Lab Monitoring
- Monitor for signs of systemic corticosteroid effects (e.g., adrenal suppression) with long-term, high-dose therapy.
- Periodically monitor serum potassium levels, especially in patients susceptible to hypokalemia.
Doctor Notes
- Emphasize proper inhaler technique and the importance of rinsing the mouth after each use to patients.
- Clearly differentiate that this medication is for maintenance therapy and not for acute symptom relief.
- Monitor patients for systemic corticosteroid effects, especially those on high doses or co-medicated with CYP3A4 inhibitors.
Patient Guidelines
- Always use your inhaler exactly as prescribed by your doctor.
- Rinse your mouth with water and spit it out after each dose to help prevent oral candidiasis (thrush).
- Formocort is for maintenance therapy and should not be used for sudden relief of asthma or COPD symptoms.
- Do not stop taking the medicine suddenly without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Generally, Formocort does not affect the ability to drive or operate machinery. However, if you experience dizziness, tremor, or blurred vision, you should exercise caution.
Lifestyle Advice
- Avoid known asthma or COPD triggers (e.g., allergens, smoke, pollution).
- Maintain a healthy lifestyle, including regular exercise and a balanced diet.
- Stop smoking, as it is crucial for managing COPD and improving asthma control.
