Description
Overview of the medicine
Benzathine Penicillin G is a long-acting form of penicillin used to treat various bacterial infections, primarily syphilis and as prophylaxis for rheumatic fever. It is administered via deep intramuscular injection.
Uses & Indications
Dosage
Adults
Syphilis: Primary, secondary, latent (<1 year) - 2.4 million units IM single dose. Latent (>1 year) or tertiary - 2.4 million units IM weekly for 3 weeks. Rheumatic fever prophylaxis: 1.2 million units IM every 3-4 weeks. Streptococcal pharyngitis: 1.2 million units IM single dose.
Elderly
Similar to adult dosage, but caution with renal impairment.
Renal_impairment
No specific dose adjustment for mild to moderate impairment, but careful monitoring is advised for severe renal impairment due to potential accumulation.
How to Take
Administer by deep intramuscular injection only, preferably in the upper outer quadrant of the buttock. Do not administer intravenously, subcutaneously, or intra-arterially.
Mechanism of Action
It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bactericidal effects against susceptible organisms.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 12-24 hours.
Excretion
Excreted primarily unchanged by the kidneys.
Half life
Approximately 7-14 days, due to slow absorption and release.
Absorption
Slowly and completely absorbed from the intramuscular injection site, providing prolonged detectable blood levels.
Metabolism
Minimal metabolism.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or any other beta-lactam antibiotics (e.g., cephalosporins)
- Intravenous administration (can cause cardio-respiratory arrest and death)
Drug Interactions
Probenecid
Decreases the renal tubular secretion of penicillin, prolonging its half-life and increasing serum concentrations.
Methotrexate
Reduced excretion of methotrexate, increasing its toxicity.
Tetracyclines
May antagonize the bactericidal effect of penicillin.
Oral Contraceptives
May reduce efficacy of oral contraceptives (though controversial).
Storage
Store below 30°C in a dry place, protected from light. Do not freeze.
Overdose
Overdose is rare due to single-dose administration. Symptoms may include neuromuscular hyperexcitability or seizures in very high doses, especially with renal impairment. Management is symptomatic and supportive, including hemodialysis if severe.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy. Penicillins are excreted in breast milk in small amounts; generally considered safe for breastfeeding infants, but monitor for allergic reactions in the infant.

- Complete blood count (CBC) if signs of hematopoietic toxicity occur
Doctor Notes
- Emphasize IM administration only, never IV, due to severe adverse reaction risk.
- Assess penicillin allergy history thoroughly before administration.
- Counsel patients on potential Jarisch-Herxheimer reaction during syphilis treatment.
Patient Guidelines
- Inform your doctor about any allergies, especially to penicillins or cephalosporins.
- Report any signs of allergic reaction (rash, itching, difficulty breathing) immediately.
- Ensure the injection is given intramuscularly only.
Missed Dose Advice
As this is usually a single or infrequent dose administered by a healthcare professional, missing a dose is unlikely. If a scheduled dose is missed, contact your doctor immediately for rescheduling.
Driving Precautions
Benzathine Penicillin G is not known to impair the ability to drive or operate machinery. However, if allergic reactions or dizziness occur, avoid such activities.
Lifestyle Advice
- Follow all medical advice and complete the full course of treatment.
