Description
Overview of the medicine
Gabantin 100 mg Tablet is used to treat epilepsy (seizures) and neuropathic pain (nerve pain). It works by decreasing the abnormal excitement in the brain.
Uses & Indications
Dosage
Adults
For neuropathic pain: Initially 300 mg on day 1, 300 mg twice daily on day 2, and 300 mg three times daily on day 3, then titrated up to a maximum of 3600 mg/day in three divided doses. For epilepsy: Initially 300 mg three times daily, adjusted according to response, up to 1200 mg/day; maximum 2400-3600 mg/day.
Elderly
Reduced dosage and careful titration advised due to potential age-related decrease in renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance. For CrCl 30-59 mL/min, maximum 700 mg/day; for CrCl 15-29 mL/min, maximum 400 mg/day.
How to Take
Gabantin 100 mg Tablet should be taken orally, with or without food. It is recommended to swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Gabapentin is thought to exert its effect by modulating the activity of voltage-gated calcium channels, particularly the alpha-2-delta subunit, reducing the release of excitatory neurotransmitters. It does not bind to GABA receptors directly.
Pharmacokinetics
Onset
Onset of action for pain relief can be days to weeks; for seizures, it's typically within days of achieving therapeutic levels.
Excretion
Excreted unchanged by the kidneys. Renal clearance is directly proportional to creatinine clearance.
Half life
Approximately 5 to 7 hours in adults with normal renal function.
Absorption
Oral absorption is dose-dependent and saturable, decreasing with increasing doses. Bioavailability ranges from 27% to 60%.
Metabolism
Not significantly metabolized in humans. It does not induce hepatic microsomal enzymes.
Side Effects
Contraindications
- Hypersensitivity to gabapentin or any component of the formulation.
Drug Interactions
Antacids
May decrease the bioavailability of gabapentin; take gabapentin at least 2 hours after antacid administration.
Opioids (e.g., Morphine)
May increase gabapentin concentrations and enhance CNS depressant effects, leading to increased sedation and respiratory depression. Monitor patients for signs of CNS depression.
CNS Depressants (e.g., Alcohol, Benzodiazepines)
Concurrent use may enhance CNS depressant effects of gabapentin. Avoid alcohol and use with caution with other CNS depressants.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, double vision, slurred speech, drowsiness, lethargy, and diarrhea. Treatment is primarily supportive. Hemodialysis can be used for severe cases, as gabapentin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Gabapentin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; caution should be exercised when gabapentin is administered to a nursing woman.

Lab Monitoring
- Monitor renal function, especially in elderly patients or those with kidney impairment, for appropriate dose adjustment. No routine specific lab monitoring is generally required for gabapentin in patients with normal renal function.
Doctor Notes
- Emphasize slow titration of gabapentin to minimize adverse effects and optimize efficacy.
- Adjust dose in renal impairment. Monitor for signs of suicidal ideation or unusual behavioral changes, especially at the initiation of therapy or during dose changes.
- Educate patients on the importance of not discontinuing gabapentin abruptly.
Patient Guidelines
- Do not stop taking Gabantin abruptly without consulting your doctor, as it may cause withdrawal seizures or worsening of symptoms.
- Report any unusual changes in mood, behavior, or suicidal thoughts to your doctor immediately.
- Avoid driving or operating heavy machinery until you know how the medicine affects you, as it may cause dizziness or drowsiness.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose to catch up.
Driving Precautions
Gabantin 100 mg Tablet may cause dizziness, drowsiness, and blurred vision. Patients should be advised not to drive or operate hazardous machinery until they are reasonably certain that gabapentin does not adversely affect their ability to engage in such activities.
Lifestyle Advice
- Avoid alcohol consumption while taking gabapentin, as it can increase drowsiness and dizziness.
- Maintain a regular dosing schedule to ensure consistent blood levels of the medication and maximize its effectiveness.
