Description
Overview of the medicine
Initer 2000 IU Injection contains Epoetin Alfa, a recombinant human erythropoietin, used to stimulate red blood cell production in patients with anemia due to chronic kidney disease, chemotherapy, or certain surgical procedures.
Uses & Indications
Dosage
Adults
CKD: Initial dose 50-100 IU/kg 3 times a week (IV/SC). Adjust based on hemoglobin response. Chemotherapy-induced anemia: 150 IU/kg 3 times a week (SC) or 40,000 IU once weekly (SC).
Elderly
No specific dose adjustment for elderly patients, but careful monitoring is advised due to potential comorbidities.
Renal_impairment
The primary indication is for anemia in chronic renal failure; dosage is adjusted to achieve target hemoglobin levels.
How to Take
Administer intravenously or subcutaneously. Do not shake the vial. The solution should be clear and colorless.
Mechanism of Action
Epoetin Alfa is a glycoprotein that stimulates erythropoiesis by binding to the erythropoietin receptor on erythroid progenitor cells, leading to their proliferation and differentiation into mature red blood cells.
Pharmacokinetics
Onset
Hemoglobin levels begin to rise within 2-6 weeks.
Excretion
Mainly through the reticuloendothelial system; minimal renal excretion.
Half life
Approximately 4-13 hours after intravenous administration; slightly longer after subcutaneous administration (13-24 hours).
Absorption
Bioavailability is 31-61% after subcutaneous administration. Intravenous administration provides immediate and complete bioavailability.
Metabolism
Primarily cleared by erythroid precursor cells.
Side Effects
Contraindications
- Uncontrolled hypertension.
- Pure Red Cell Aplasia (PRCA) unrelated to erythropoietin.
- Known hypersensitivity to Epoetin Alfa or any excipients.
Drug Interactions
Cyclosporine
May require increased cyclosporine dose due to decreased blood levels.
Iron supplements
Essential for optimal response; inadequate iron can limit efficacy.
Storage
Store in a refrigerator (2°C to 8°C). Do not freeze. Protect from light.
Overdose
Overdose can lead to symptoms of polycythemia (e.g., increased blood viscosity). Management includes phlebotomy if clinically indicated.
Pregnancy & Lactation
Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution when administering to a nursing mother.
