Description
Overview of the medicine
Lansec 30 mg Capsule contains Lansoprazole, which is a proton pump inhibitor (PPI). It works by decreasing the amount of acid produced in the stomach, helping to relieve symptoms and heal acid-related damage to the esophagus, stomach, and intestines.
Uses & Indications
Dosage
Adults
For duodenal ulcer: 30 mg once daily for 4 weeks. For gastric ulcer: 30 mg once daily for 8 weeks. For reflux esophagitis: 30 mg once daily for 4-8 weeks. For H. pylori eradication: 30 mg twice daily for 7-14 days (in combination therapy).
Elderly
Generally no dose adjustment needed. However, caution in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take the capsule orally, preferably in the morning before breakfast. Swallow whole with water; do not crush or chew the capsule. For patients who have difficulty swallowing, the capsule may be opened and the intact granules sprinkled on a spoonful of applesauce or yogurt.
Mechanism of Action
Lansoprazole is a proton pump inhibitor that suppresses gastric acid secretion by irreversibly inhibiting the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells. This reduces both basal and stimulated acid secretion.
Pharmacokinetics
Onset
Acid suppression begins within 1-2 hours.
Excretion
Approximately one-third of the dose is excreted in the urine, and two-thirds in the feces (primarily as metabolites).
Half life
Plasma elimination half-life is 1.3 to 2 hours, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is about 80-90%. Peak plasma concentrations are reached within 1.5-2.2 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzymes CYP2C19 and CYP3A4. The main metabolites are inactive.
Side Effects
Contraindications
- Hypersensitivity to lansoprazole or any component of the formulation.
- Co-administration with atazanavir.
Drug Interactions
Digoxin
May increase digoxin absorption and levels.
Warfarin
Possible increased INR and bleeding risk.
Atazanavir
Significantly reduces plasma concentrations of atazanavir, contraindicating co-administration.
Iron Salts
May reduce absorption of iron salts due to increased gastric pH.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
May increase methotrexate levels, especially with high-dose methotrexate.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include lethargy, tremor, convulsions, and hypothermia. Management is symptomatic and supportive. Lansoprazole is not significantly removed from the circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is unknown whether lansoprazole is excreted in human milk; caution should be exercised when administered to a nursing mother.
