Description
Overview of the medicine
Ligazid MX 5 mg Tablet contains Gliclazide (Modified Release), an oral antidiabetic drug belonging to the sulfonylurea class. It is used to control blood sugar levels in patients with Type 2 Diabetes Mellitus when diet and exercise alone are not sufficient. The modified release formulation ensures a sustained therapeutic effect throughout the day.
Uses & Indications
Dosage
Adults
The usual starting dose of Gliclazide MR is typically 30 mg once daily. For a hypothetical 5 mg MR formulation, the initial dose may be 5 mg once daily, adjusted based on blood glucose levels and clinical response. Maximum dose for Gliclazide MR is generally 120 mg once daily.
Elderly
Similar to adults, but with careful monitoring of blood glucose to avoid hypoglycemia. Initial dose may be lower.
Renal_impairment
Mild to moderate renal impairment usually does not require dose adjustment. Severe renal impairment (CrCl < 30 mL/min) is generally contraindicated due to increased risk of hypoglycemia. Consult physician.
How to Take
Ligazid MX 5 mg Tablet should be taken orally once daily with breakfast. Swallow the tablet whole with water, do not crush or chew. Taking it with a meal helps reduce gastrointestinal side effects and the risk of hypoglycemia.
Mechanism of Action
Gliclazide stimulates insulin secretion from the pancreatic beta cells by binding to sulfonylurea receptors on the cell surface. This binding leads to the closure of ATP-sensitive potassium channels, causing depolarization and subsequent calcium influx, which triggers insulin release. It also has hemovascular effects, reducing platelet aggregation and improving fibrinolysis.
Pharmacokinetics
Onset
Onset of action typically within a few hours.
Excretion
Primarily excreted in urine (less than 1% unchanged) and feces.
Half life
Approximately 12-20 hours for the modified release formulation.
Absorption
Well absorbed after oral administration. The modified release formulation ensures gradual and sustained absorption.
Metabolism
Extensively metabolized in the liver by CYP2C9.
Side Effects
Contraindications
- Hypersensitivity to gliclazide or other sulfonylureas
- Type 1 Diabetes Mellitus
- Diabetic ketoacidosis
- Severe renal or hepatic impairment
- Concomitant treatment with miconazole
Drug Interactions
Alcohol
Increases the hypoglycemic effect, can lead to severe hypoglycemia.
Miconazole
Increases the hypoglycemic effect of gliclazide, potentially leading to severe hypoglycemia. Concomitant use is contraindicated.
Beta-blockers
May mask symptoms of hypoglycemia and potentiate hypoglycemic effect.
Phenylbutazone (systemic route)
Increases the hypoglycemic effect of gliclazide (displaces protein binding), consider another anti-inflammatory agent.
Corticosteroids, Diuretics, Danazol
May decrease the hypoglycemic effect of gliclazide.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of Gliclazide can lead to severe hypoglycemia. Symptoms include sweating, shakiness, confusion, rapid heartbeat, hunger, and in severe cases, seizures, coma. Management involves immediate administration of glucose (oral glucose for conscious patients, IV glucose for unconscious patients) and careful monitoring of blood glucose levels for at least 24-48 hours. Hospitalization may be required.
Pregnancy & Lactation
Gliclazide is generally not recommended during pregnancy. Insulin is preferred for glycemic control during pregnancy. It is unknown whether gliclazide is excreted in human milk, so it is generally not recommended during breastfeeding.

Lab Monitoring
- Regular monitoring of blood glucose levels (fasting and post-prandial)
- HbA1c levels every 3-6 months
- Renal function (creatinine, eGFR) periodically
- Liver function tests (ALT, AST) periodically
Doctor Notes
- Counsel patients on hypoglycemia symptoms and management.
- Emphasize lifestyle modifications (diet, exercise).
- Monitor renal and hepatic function, especially in elderly patients.
- Avoid co-administration with miconazole due to high risk of severe hypoglycemia.
Patient Guidelines
- Take your medicine regularly as prescribed by your doctor.
- Do not skip meals and try to maintain a consistent meal schedule.
- Monitor your blood glucose levels regularly as instructed.
- Be aware of symptoms of hypoglycemia and how to treat them (e.g., carry glucose tablets or candy).
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Gliclazide can cause hypoglycemia, which may impair your ability to drive or operate machinery. Patients should be aware of the symptoms of hypoglycemia and take precautions before driving, especially when initiating treatment or adjusting dosage.
Lifestyle Advice
- Follow a healthy, balanced diet with controlled carbohydrate intake.
- Engage in regular physical activity as recommended by your doctor.
