Dosage
Adults
Initial dose is 0.5 mg to 1 mg taken 15-30 minutes before each main meal. Dose may be adjusted based on blood glucose response, up to a maximum of 4 mg per meal.
Elderly
No specific dose adjustment is generally required, but close monitoring of blood glucose is recommended due to potential for increased sensitivity to hypoglycemia.

How to Take
Take the tablet orally 15-30 minutes before each main meal. Swallow whole with water; do not crush or chew. If a meal is skipped, the dose of Limpet-M should also be skipped.
Mechanism of Action
Metiaglinide is a meglitinide analogue that stimulates insulin secretion from pancreatic beta cells. It binds to specific receptors on beta cell membranes, leading to the closure of ATP-sensitive potassium channels. This depolarization opens calcium channels, resulting in increased intracellular calcium and subsequent insulin release.

Side Effects
Contraindications

Drug Interactions
Gemfibrozil
May significantly increase plasma concentrations of Metiaglinide, increasing the risk of hypoglycemia. Co-administration is not recommended.

These CYP3A4 inducers may decrease Metiaglinide levels, leading to reduced efficacy.
Clarithromycin, Itraconazole, Ketoconazole
