Micoderm-HC 2/1 Cream is a combination topical medication containing Miconazole Nitrate (an antifungal) and Hydrocortisone (a corticosteroid). It is used to treat fungal skin infections that are accompanied by inflammation, redness, and itching.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area and surrounding skin once or twice daily, depending on the severity of the condition, for 2-4 weeks or as directed by the physician.
Elderly
No specific dosage adjustment is required for elderly patients. Use as for adults.
Renal_impairment
No dosage adjustment is necessary for topical application in patients with renal impairment.
How to Take
For external use only. Wash and dry the affected area thoroughly before application. Apply a thin layer of cream to the affected skin and gently rub it in until it disappears. Wash hands after use unless hands are the treated area. Do not cover the treated area with occlusive dressings unless advised by a doctor.
Mechanism of Action
Miconazole Nitrate inhibits the biosynthesis of ergosterol, a vital component of fungal cell membranes, leading to increased permeability and eventual cell death. Hydrocortisone, a mild corticosteroid, exerts anti-inflammatory, antipruritic, and vasoconstrictive actions by inhibiting the release of inflammatory mediators.

Onset
Symptomatic relief of itching and inflammation may be noticed within a few days of starting treatment, with complete resolution of fungal infection typically taking 2-4 weeks.
Excretion
Systemically absorbed components are excreted via urine and feces.
Half life
Systemic half-life data is not clinically significant for topical application due to very low systemic absorption.
Absorption
Minimal systemic absorption of both miconazole and hydrocortisone occurs after topical application to intact skin. Absorption may increase with skin damage, occlusive dressings, or application to large surface areas.
Metabolism
Any systemically absorbed miconazole is metabolized in the liver. Hydrocortisone is also metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to miconazole, hydrocortisone, other azole antifungals, corticosteroids, or any excipients of the cream.
- Viral infections of the skin (e.g., herpes simplex, varicella).
- Tuberculosis of the skin.
- Acne rosacea, perioral dermatitis, ulcerative conditions of the skin.
Drug Interactions
Warfarin
Although systemic absorption is minimal, theoretically, miconazole can enhance the anticoagulant effect of warfarin. Monitor INR if used concurrently, especially on large surface areas or prolonged use.
Storage
Store below 30°C in a dry place, away from direct sunlight. Do not freeze. Keep out of reach of children.
Overdose
Overdose from topical application is unlikely due to minimal systemic absorption. In case of accidental ingestion, supportive measures should be taken and medical attention sought immediately.
