Overview of the medicine
Miki-H 2-1 Cream is a combination topical medication containing Miconazole Nitrate (an antifungal) and Hydrocortisone (a corticosteroid). It is used to treat fungal skin infections accompanied by inflammation, itching, and redness.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected area twice daily (morning and evening) for 2-4 weeks, or as directed by physician. Continue treatment for at least 7 days after symptoms disappear.
Elderly
No specific dosage adjustment required. Use with caution for prolonged periods or on large areas.
Renal_impairment
No specific dosage adjustment required for topical use.
How to Take
For external use only. Wash and dry the affected area before applying the cream. Apply a thin layer and gently rub it into the skin. Wash hands immediately after application.
Mechanism of Action
Miconazole inhibits the biosynthesis of ergosterol, an essential component of fungal cell membranes, leading to increased cell membrane permeability and leakage of cellular contents. Hydrocortisone is a corticosteroid that reduces inflammation, itching, and vasoconstriction by inhibiting the release of inflammatory mediators.

Minimal systemic absorption through intact skin. Absorption may increase with inflammation, occlusion, or application to large surface areas. Miconazole: <1%. Hydrocortisone: 1-5%.
Metabolism
Absorbed Miconazole is metabolized in the liver; Hydrocortisone is metabolized locally in the skin and systemically in the liver.

Drug Interactions
Warfarin
Miconazole, even topical, may enhance the anticoagulant effect of warfarin, leading to increased bleeding risk. Monitor INR.
Other corticosteroids
Concurrent use of other corticosteroids may increase the risk of systemic corticosteroid side effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Accidental ingestion is unlikely to cause serious effects, but gastric lavage or emesis may be considered if large quantities are ingested. Excessive topical application may lead to local skin irritation or, in rare cases of prolonged use over large areas, systemic corticosteroid effects. Discontinue use and treat symptomatically.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid prolonged use or large areas. Exercise caution when used by nursing mothers; avoid application to the breast area to prevent infant exposure.
