Description
Overview of the medicine
Motilon 1 mg Pediatric Drop contains Domperidone, an antiemetic and prokinetic agent used to treat nausea, vomiting, and certain gastrointestinal motility disorders in children. It works by blocking dopamine receptors.
Uses & Indications
Dosage
Adults
Not typically used as a primary formulation for adults; consult physician for appropriate adult formulations and dosage.
Elderly
Use with caution and at the lowest effective dose due to potential cardiac risks. Not the primary formulation.
Renal_impairment
Dosage reduction may be necessary in severe renal impairment. Consult a physician. Not the primary formulation.
How to Take
Administer orally using the calibrated dropper provided, preferably 15-30 minutes before meals. Do not exceed the prescribed dose or duration.
Mechanism of Action
Domperidone acts as a peripheral dopamine D2 receptor antagonist, primarily in the chemoreceptor trigger zone (CTZ) and the gastrointestinal tract. This action increases gastrointestinal motility and blocks the emetic response.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Approximately 66% excreted in feces and 33% in urine as inactive metabolites.
Half life
7-9 hours (prolonged in severe renal impairment)
Absorption
Rapidly absorbed after oral administration; low absolute bioavailability (approx. 15%) due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver via CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Domperidone or any component of the formulation.
- Prolactin-releasing pituitary tumor (prolactinoma).
- Moderate to severe hepatic impairment.
- Conditions where increased gastrointestinal motility could be harmful (e.g., gastrointestinal hemorrhage, mechanical obstruction, perforation).
- Known existing prolongation of cardiac conduction intervals (e.g., QT interval).
- Significant electrolyte disturbances (hypokalemia, hypomagnesemia).
- Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin, fluconazole, amiodarone, diltiazem, verapamil).
- Concomitant use with other QT-prolonging drugs.
Drug Interactions
Antacids, H2-receptor antagonists (e.g., Cimetidine, Ranitidine)
May decrease the oral bioavailability of domperidone. Should not be taken simultaneously; administer domperidone before meals and antacids/H2-blockers after meals.
Potent CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin, Clarithromycin)
Significantly increases plasma levels of domperidone, increasing the risk of QT prolongation. Concomitant use is contraindicated.
Other QT-prolonging drugs (e.g., some antiarrhythmics, antipsychotics, antidepressants)
Additive effect on QT interval, increasing the risk of arrhythmias. Concomitant use is contraindicated.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, disorientation, extrapyramidal reactions, and possibly cardiac arrhythmias. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal. ECG monitoring is recommended.
Pregnancy & Lactation
Limited data on use in pregnancy; should be used only if potential benefits justify the potential risk to the fetus. Domperidone is excreted in breast milk and may affect the infant; avoid during breastfeeding if possible or consider alternative treatment.

Lab Monitoring
- ECG monitoring should be considered in patients at risk of cardiac arrhythmias.
- Renal function monitoring in patients with renal impairment.
Doctor Notes
- Emphasize the importance of accurate dosing, especially in pediatric patients, to minimize risks.
- Counsel parents/guardians about the cardiac risks associated with Domperidone and the need to strictly adhere to the prescribed dose and duration (max 7 days).
- Avoid concomitant use with potent CYP3A4 inhibitors and other QT-prolonging drugs. Review patient's full medication list.
- Consider ECG in patients with pre-existing cardiac conditions or those at higher risk.
Patient Guidelines
- Administer the drops with the provided calibrated dropper for accurate dosing.
- Take the medicine 15-30 minutes before meals for best results.
- Do not exceed the prescribed dose or duration of treatment (usually not more than 7 days).
- Consult a doctor if symptoms do not improve or worsen.
- Be aware of potential cardiac risks and report any signs of heart problems (e.g., palpitations, fainting) to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
