Description
Overview of the medicine
Neuratam 500 mg Syrup contains Piracetam, a nootropic agent from the racetam group. It is believed to improve cognitive function, memory, learning ability, and attention by influencing neural and vascular functions.
Uses & Indications
Dosage
Adults
Initial dose: 2.4 g/day in 2-3 divided doses. Maintenance dose: 1.2-4.8 g/day in 2-3 divided doses. For syrup (500mg/5ml), this translates to 10-40 ml daily.
Elderly
No specific dose adjustment for elderly, but monitor renal function. Start with a lower dose.
Renal_impairment
Creatinine clearance 50-79 ml/min: 2/3 of usual daily dose in 2-3 divided doses. Creatinine clearance 30-49 ml/min: 1/3 of usual daily dose in 2 divided doses. Creatinine clearance <30 ml/min or end-stage renal disease: Contraindicated.
How to Take
Take orally, with or without food. The daily dose should be divided into 2 or 3 doses. May be mixed with water or other liquids.
Mechanism of Action
Piracetam enhances cognitive function by various mechanisms, including improving the fluidity of neuronal membranes, increasing cerebral blood flow, and modulating neurotransmission (e.g., acetylcholine, glutamate). It also enhances mitochondrial activity and ATP synthesis in brain cells.
Pharmacokinetics
Onset
Cognitive effects may be noticeable within hours to days, but full therapeutic benefits can take several weeks.
Excretion
Excreted primarily unchanged by the kidneys, with approximately 80-100% of the dose recovered in urine within 24-30 hours.
Half life
Plasma half-life is approximately 4-5 hours; CSF half-life is 6-8 hours.
Absorption
Rapid and complete absorption from the gastrointestinal tract. Peak plasma concentrations are achieved within 0.5-1 hour after oral administration.
Metabolism
Piracetam is not metabolized in the human body.
Side Effects
Contraindications
- Hypersensitivity to piracetam or any component of the formulation
- Severe renal impairment (creatinine clearance <20 ml/min)
- Cerebral hemorrhage
- Huntington's chorea
Drug Interactions
CNS depressants
May enhance CNS depressant effects when co-administered.
Thyroid hormones
Concomitant use with thyroid extracts can increase the risk of confusion, irritability, and sleep disorders.
Anticoagulants (e.g., Warfarin)
Piracetam may potentiate the effects of anticoagulants, requiring careful monitoring of prothrombin time/INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Piracetam has a wide margin of safety. Symptoms of overdose are generally mild and may include gastrointestinal discomfort, dizziness, or nervousness. Treatment is symptomatic and supportive; no specific antidote exists.
Pregnancy & Lactation
Pregnancy Category B/C. Avoid use during pregnancy unless the potential benefit outweighs the potential risk to the fetus. Not recommended during breastfeeding as piracetam is excreted in breast milk.

Lab Monitoring
- Renal function tests (especially in elderly or patients with pre-existing renal impairment)
Doctor Notes
- Assess renal function before initiating and periodically during piracetam therapy.
- Counsel patients on potential central nervous system side effects (e.g., nervousness, insomnia) and the importance of not discontinuing abruptly.
- Consider drug interactions, especially with anticoagulants and thyroid hormones.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking Piracetam suddenly, as it may lead to withdrawal symptoms, especially in myoclonus.
- It may take several weeks to see the full therapeutic benefits.
- Inform your doctor about all other medicines you are taking, including herbal supplements.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
