Description
Overview of the medicine
Nictoris 5 mg Tablet contains Solifenacin Succinate, an anticholinergic medication used to treat symptoms of overactive bladder (OAB), such as urinary urgency, frequency, and incontinence.
Uses & Indications
Dosage
Adults
The recommended starting dose is 5 mg once daily. If needed, the dose may be increased to 10 mg once daily based on efficacy and tolerability.
Elderly
No dose adjustment is required for elderly patients.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), the dose should not exceed 5 mg once daily. Use with caution in moderate hepatic impairment, dose not to exceed 5 mg daily. Contraindicated in severe hepatic impairment.
How to Take
Nictoris tablet should be taken orally, swallowed whole with liquid, and can be taken with or without food.
Mechanism of Action
Solifenacin is a competitive muscarinic receptor antagonist, specifically M3 selective. By blocking M3 receptors in the bladder, it inhibits the contraction of the detrusor muscle, thereby increasing bladder capacity and reducing involuntary contractions associated with overactive bladder.
Pharmacokinetics
Onset
Clinical effects may be observed within a few weeks, with full effect achieved within 2-4 weeks.
Excretion
Approximately 69% of the dose is excreted in the urine and 22% in the feces.
Half life
Approximately 45-68 hours.
Absorption
Well absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 90%. Peak plasma concentration is reached within 3 to 8 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 3A4 (CYP3A4) and to a lesser extent by CYP2D6.
Side Effects
Contraindications
- Patients with urinary retention.
- Gastric retention.
- Uncontrolled narrow-angle glaucoma.
- Myasthenia gravis.
- Severe hepatic impairment.
- Severe renal impairment receiving strong CYP3A4 inhibitors.
- Known hypersensitivity to solifenacin or any component of the formulation.
Drug Interactions
Cholinergic Agonists
May reduce the effect of solifenacin.
Drugs that prolong QT interval
Use with caution due to potential for QT prolongation at high doses.
CYP3A4 Inducers (e.g., Rifampicin, Phenytoin)
May decrease solifenacin exposure, potentially reducing efficacy.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Concomitant use with strong CYP3A4 inhibitors may increase solifenacin exposure. The dose of Nictoris should not exceed 5 mg daily.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe anticholinergic effects such as dry mouth, blurred vision, constipation, urinary retention, tachycardia, and hallucinations. Treatment should be symptomatic and supportive. Gastric lavage and activated charcoal may be considered. In severe cases, physostigmine or other cholinergic agonists may be administered under close medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk; therefore, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.

Clinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of solifenacin in treating overactive bladder symptoms. Trials have shown significant reductions in incontinence episodes, urgency, and micturition frequency.
Lab Monitoring
- No specific routine laboratory monitoring is required. However, renal and hepatic function should be assessed before initiating treatment and periodically in patients with pre-existing impairment.
Doctor Notes
- Counsel patients on anticholinergic side effects, particularly dry mouth and constipation, and strategies to manage them.
- Assess for risk factors for urinary retention (e.g., bladder outlet obstruction) before initiating therapy.
- Monitor for signs of cognitive impairment in elderly patients, especially during dose escalation.
Patient Guidelines
- Take this medicine exactly as prescribed by your doctor.
- Do not chew or crush the tablet; swallow it whole with water.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
- Be aware of potential side effects like dry mouth and blurred vision.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Nictoris may cause blurred vision, dizziness, and somnolence. Patients should be cautioned about engaging in activities requiring mental alertness, such as driving or operating machinery, until they know how the drug affects them.
Lifestyle Advice
- Limit consumption of bladder irritants such as caffeine, alcohol, and spicy foods.
- Practice bladder training techniques to gradually increase the time between urinations.
- Maintain a healthy weight and engage in regular physical activity.
