Overview of the medicine
Olpres-HZ 20 mg Tablet is a combination medicine containing Olmesartan medoxomil, an angiotensin II receptor blocker, and Hydrochlorothiazide, a thiazide diuretic. It is primarily used to treat high blood pressure (hypertension) when monotherapy is not sufficient. This combination helps to lower blood pressure by relaxing blood vessels and increasing the excretion of salt and water from the body.
Uses & Indications
Dosage
Adults
Usually 1 tablet once daily. The dose can be adjusted based on individual response and tolerability, typically after 2-4 weeks of therapy.
Elderly
No specific dose adjustment is usually required for elderly patients with normal renal function. Close monitoring is advised.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min). For moderate impairment, close monitoring of potassium and creatinine levels is recommended.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with a glass of water. It is advisable to take the medicine at the same time each day.
Mechanism of Action
Olmesartan medoxomil selectively blocks the AT1 angiotensin II receptor, preventing vasoconstriction and aldosterone release. Hydrochlorothiazide is a thiazide diuretic that acts on the renal distal convoluted tubule to inhibit sodium and chloride reabsorption, leading to increased urinary excretion of water and electrolytes, thereby reducing blood volume and blood pressure.
Pharmacokinetics
Onset
Blood pressure reduction is noticeable within approximately 2 hours for both components, with the full antihypertensive effect developing over 1-2 weeks for olmesartan.
Excretion
Olmesartan is eliminated via biliary excretion (50-65%) and renal excretion (35-50%). Hydrochlorothiazide is predominantly eliminated unchanged by renal excretion.
Half life
The elimination half-life of olmesartan is approximately 13 hours. The half-life of hydrochlorothiazide is between 5.6 and 14.8 hours.
Absorption
Olmesartan medoxomil is rapidly and completely converted to active olmesartan during absorption from the gastrointestinal tract. Its absolute bioavailability is approximately 26%. Hydrochlorothiazide is rapidly absorbed from the gastrointestinal tract with a bioavailability of 60-80%.
Metabolism
Olmesartan is not metabolized by the cytochrome P450 system; it is almost entirely excreted unchanged. Hydrochlorothiazide is not metabolized and is eliminated rapidly by the kidney.

Contraindications
- Anuria
- Hypersensitivity to olmesartan, hydrochlorothiazide, or sulfonamide-derived drugs
- Severe renal impairment
- Severe hepatic impairment
- Pregnancy (second and third trimesters)
Drug Interactions
NSAIDs
Reduced antihypertensive effect and increased risk of renal impairment, especially in dehydrated patients. Monitor renal function.
Lithium
Increased serum lithium concentrations and lithium toxicity. Monitor serum lithium levels.
Alcohol/Barbiturates/Narcotics
Potentiation of orthostatic hypotension.
Cholestyramine/Colestipol resins
May impair absorption of hydrochlorothiazide. Administer these agents separately.
Insulin/Oral hypoglycemic agents
Dosage adjustment of antidiabetic agents may be required as thiazides can decrease glucose tolerance.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia. Avoid concomitant use unless closely monitored.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hypotension, tachycardia, bradycardia (due to vagal stimulation), and electrolyte disturbances (hypokalemia, hyponatremia). Treatment is symptomatic and supportive. Induce emesis or perform gastric lavage if ingestion is recent. Administer activated charcoal. Monitor vital signs and electrolytes.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to risk of fetal injury or death. Discontinue if pregnancy is detected. Not recommended during lactation as both components are excreted in breast milk.
