Pantex 40 mg tablet contains Pantoprazole, which is a proton pump inhibitor used to treat conditions involving excess stomach acid. It works by irreversibly blocking the proton pumps in the stomach lining, thereby reducing acid production.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Maintenance: 20-40 mg daily. Duodenal/Gastric Ulcer: 40 mg once daily for 2-4 weeks. H. pylori eradication: 40 mg twice daily for 7-14 days (with antibiotics). Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted as needed.
Elderly
No dosage adjustment generally required, but close monitoring is advisable.
Renal_impairment
No dosage adjustment required.
Hepatic_impairment
For severe hepatic impairment, a maximum dose of 20 mg once daily may be considered.
How to Take
Take the tablet whole with a glass of water, usually 30-60 minutes before a meal. Do not crush, chew, or break the tablet.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inhibits gastric acid secretion by irreversibly binding to the H+/K+-ATPase enzyme (proton pump) in the secretory canaliculi of the gastric parietal cells. This action blocks the final step in acid production, regardless of the stimulus, leading to a profound and long-lasting inhibition of gastric acid secretion.

Onset of action within 2.5 hours.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 18%) as metabolites.
Half life
Approximately 1 hour (terminal elimination half-life is longer, up to 7-9 hours after repeated dosing due to sustained acid suppression).
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 77% and is not affected by food.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or any component of the formulation
- Known hypersensitivity to substituted benzimidazoles
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Methotrexate
Increased and prolonged elevation of methotrexate levels, especially with high doses.
Atazanavir, Nelfinavir
Reduced absorption, avoid co-administration.
Ketoconazole, Itraconazole, Erlotinib
Absorption depends on gastric pH, may reduce bioavailability.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be initiated. Pantoprazole is not significantly removed by hemodialysis. There is no specific antidote.
