Description
Overview of the medicine
A-Clox 500 mg Injection is a combination antibiotic used to treat a wide range of bacterial infections, especially those caused by beta-lactamase producing strains. It contains Ampicillin, a broad-spectrum penicillin, and Cloxacillin, a penicillinase-resistant penicillin.
Uses & Indications
Dosage
Adults
500 mg to 1 g (Ampicillin + Cloxacillin) every 6 hours by IM or slow IV injection. The dose may be increased in severe infections.
Elderly
Similar to adult dosage, but caution should be exercised and dosage adjustment may be necessary in patients with impaired renal function.
Renal_impairment
Dose reduction is required based on creatinine clearance. For CrCl < 10 mL/min, extend dosing interval to 8-12 hours.
How to Take
For IM administration, reconstitute the powder with sterile water for injection. For IV administration, reconstitute and then dilute with a suitable intravenous fluid (e.g., normal saline, dextrose). Administer slowly over 3-5 minutes for IV push or infuse over 30-60 minutes.
Mechanism of Action
Both Ampicillin and Cloxacillin act by inhibiting bacterial cell wall synthesis, leading to cell lysis and bacterial death. Ampicillin provides broad-spectrum activity against many Gram-positive and Gram-negative bacteria, while Cloxacillin adds resistance to degradation by bacterial beta-lactamase enzymes.
Pharmacokinetics
Onset
Rapid onset, typically within minutes for IV administration and 15-30 minutes for IM.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 0.5-1.5 hours for both components in individuals with normal renal function.
Absorption
Rapidly absorbed after intramuscular (IM) injection. Intravenous (IV) administration achieves immediate systemic concentrations.
Metabolism
Minimal hepatic metabolism, primarily excreted unchanged.
Side Effects
Contraindications
- Known hypersensitivity to penicillins, cephalosporins, or any component of the formulation.
- Patients with a history of penicillin-associated cholestatic jaundice/hepatic dysfunction.
Drug Interactions
Probenecid
Increases and prolongs plasma levels of ampicillin and cloxacillin by competing for renal tubular secretion.
Tetracyclines
May antagonize the bactericidal effect of penicillin antibiotics.
Anticoagulants
May potentiate the effect of anticoagulants (e.g., warfarin) by altering gut flora and reducing vitamin K synthesis.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; additional contraceptive methods should be considered.
Storage
Store the dry powder below 30°C, protected from light and moisture. Do not freeze. Refer to the manufacturer's instructions for the storage of the reconstituted solution.
Overdose
Overdose may lead to neurological toxicities including seizures, especially in patients with impaired renal function. Management is primarily symptomatic and supportive. Hemodialysis may be useful in enhancing elimination.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy, but should be used only if clearly needed. Both ampicillin and cloxacillin are excreted in breast milk; caution is advised when administered to a nursing mother.

Lab Monitoring
- Monitor renal function (creatinine, BUN) periodically, especially in patients with pre-existing renal impairment or during prolonged therapy.
- Complete blood count (CBC) for prolonged therapy.
- Liver function tests (ALT, AST) if hepatic dysfunction is suspected or during long-term treatment.
Doctor Notes
- Always perform culture and sensitivity testing prior to initiating treatment, if feasible.
- Monitor renal function, especially in elderly or renally impaired patients, and adjust dosage accordingly.
- Advise patients about the importance of completing the full course of therapy to prevent resistance.
- Be vigilant for signs of hypersensitivity reactions, including anaphylaxis.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if symptoms improve earlier.
- Report any severe or persistent side effects to your doctor immediately.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
- Do not share this medicine with others, even if they have similar symptoms.
Missed Dose Advice
If a dose is missed, administer it as soon as remembered, unless it is almost time for the next dose. In that case, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
A-Clox 500 mg Injection is not known to impair the ability to drive or operate machinery. However, if you experience dizziness or severe allergic reactions, avoid such activities.
Lifestyle Advice
- Maintain good hygiene to prevent further infections.
