Description
Overview of the medicine
Acunac-009 eye drop is an ophthalmic solution containing Ketorolac Tromethamine, a nonsteroidal anti-inflammatory drug (NSAID). It is used to reduce inflammation and pain in the eyes, particularly after cataract surgery.
Uses & Indications
Dosage
Adults
For seasonal allergic conjunctivitis: 1 drop into the affected eye(s) four times a day. For postoperative inflammation: 1 drop into the affected eye(s) four times a day, beginning 24 hours after surgery and continuing for up to two weeks.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for ophthalmic use in patients with renal impairment.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to create a pouch, and instill one drop. Do not touch the dropper tip to any surface to avoid contamination. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Ketorolac tromethamine inhibits prostaglandin synthesis by irreversibly inhibiting cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes. Prostaglandins are mediators of inflammation and pain. By reducing prostaglandin levels, Acunac-009 helps to alleviate ocular inflammation and associated pain.
Pharmacokinetics
Onset
Onset of analgesic and anti-inflammatory action can be expected within an hour of administration.
Excretion
Mainly excreted in the urine, with a small portion excreted in feces.
Half life
The systemic half-life is approximately 5-6 hours, but local therapeutic effect in the eye is more relevant.
Absorption
Minimal systemic absorption occurs after ophthalmic administration. Peak plasma concentrations are very low (ng/mL range) and generally undetectable.
Metabolism
Systemically absorbed ketorolac is extensively metabolized in the liver, primarily via glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to Ketorolac Tromethamine or any component of the formulation.
- Patients with a history of hypersensitivity to other NSAIDs (e.g., aspirin, ibuprofen) where symptoms of asthma, acute rhinitis, urticaria, angioedema, or bronchospasm have occurred.
- Concurrent use with other topical ocular NSAIDs.

Concurrent use may increase the risk of adverse effects.
Topical Ocular Corticosteroids
May impair healing or exacerbate corneal complications, especially in predisposed patients. Use with caution.
Storage
Store at room temperature below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Ophthalmic overdose is unlikely to cause systemic toxicity due to minimal systemic absorption. If accidentally ingested, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Pregnancy Category C (first two trimesters) and D (third trimester). Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman, as systemic absorption is minimal but possible.
